Adderall for MS Fatigue

MS, 22 years, Provigil / Nuvigil just not effective > 2 hrs, what is next ?  Adderall ?    I shut down around 1-2 PM, having an effect on my work quality, and this NYC heat is not helping.  It has never been this bad.  Would a next step med be Adderall, Ritilan, etc.  Should I be afraid to take due to addiction issues with those meds ?  I also take pain meds for MS pain (Opana / Norco / Lyrica), which are narcotic based (2 of them), I do not have any addiction issues at all, never abuse my meds, take minimum dose no matter the pain level, just worried about being dependent on another controlled substance that I cannot just stop taking.  With Nuvigil I do not take on weekends, as I can rest up more frequently, can I do the same with Adderall ?  just stop taking for a day or two without the worry of W/Ds ?

People stop and start stimulant meds all the time.  They are out of your system fairly quickly.  There is no tapering off period like there is for many meds.  For people with ADHD, who follow their prescription there is no addiction problems or W/Ds.   The problem for you would be if you have to keep upping your dose - although a good doc would not let that happen.
The stimulant medications are used by people with narcolepsy for long periods of time.  The military has certainly used it for a long time.
I would think/hope that your MS doc has some insights into this.  You cannot be the only one in the world to have these challenges.   So talk with your doctor.   If he agrees, he should start you out low and go slow.  So don't expect overnight results.  What you want is the minimum effective dose.  Fortunately, there are several short acting meds that would be a good starting point.
If you decide to go this route - please let us know how it works.  Best wishes.
