i have neen unable to fill out the
adult function form for social
security disability benefits and ive
been holding onto it for at least 6
months. its the main reason ive been
denied twice. i have severe panic attacks and
ive got big issues with meeting new people
and being out in public places. please can
anyone help me fill this out???

There should even be free service either from gov't or charitable organizations that can help you with this.

There are people and companies that specialize in helping people file for disability and going to bat for them. It seems to me that you should be able to find such a resource that can fill out the form for you and gather the info they need from you via telephone. Try googling "disability advocates" in your city. Best wishes!
