Testing for cause of thyroid condition

Hello nurse.  I have had hypothyroid symptoms for about a decade or more, with family history of thyroid problems.  My TSH has been nearly borderline for years.  Only this month has my TSH gone OVER the MARK into subclinical hypothyroid zone.  I'm soon to see my doctor and hope for further testing, hypo diagnosis and thyroid medicine.

My question is, what conditions/diseases could be causing this and what are all the tests that could narrow down the cause?

Sorry, I should just post my numbers:


March 2010
TSH    4.77 H     (ref 0.47-4.68)
FT4     0.86        (ref 0.60-2.19)

I said subclinical hypothyroid, but considering the new 0.3-3.0 range, I guess that is regular hypothyroid?

Sorry, also forgot to mention that several months ago my Free T3 was normal in the upper range, and then and this month my Free T4 has been very low-normal.
