You husband (if it is the poster called Foster) has spine disease in the cervical and lumbar region, and pain is consistent with his disease.
Your best bet is to find him board certified interventional pain physician. Their training gives them a special knowledge of pain related to the spine.
Surgeons will look at that report and not understand reasons for pain. They are nuts and bolts, cause and effect kind of thinkers, and are not trained in referred pain, or central pain syndromes. Also, they do a lot of cutting, which causes scar tissue that leads to additional pain.

My husband mentioned this and then today after cutting the grass it happened again. He said it is excruciating. Like most of the others he played contact sports, Lacrosse in his case, most of his life. Lifted weights for many years but has not in a while and is carrying extra weight in the mid section. Thank you all for posting I am going to get him to see a dr ASAP.
Just an fyi- he also has a low testosterone count which is common for men who have had head injuries. Just thought I would mention it incase it is any kind of missing link to all this.
