Back Pain

I just had another MRI today and I now have more problems. Here's the results copied and pasted. What's all this mean now? I was diagnosed with Arachnoiditis back in 2001.

Impression
IMPRESSION
Posterior spinal fusion again seen at L5-S1.
There is now grade 1 anterolisthesis of L4 relative to L5 and slight retrolisthesis of L3 relative to L4.
Multilevel disc bulging with mild canal narrowing at L3-4 and L4-5 with neural foraminal narrowing at L3-4 and L4-5.

Narrative
EXAM
MRI L SPINE W WO CONTRAST-9/29/2020 3:12 pm

HISTORY
Lumbar degenerative disc disease, failed back syndrome has had 2 back surgeries on chronic narcotics

COMPARISON
MRI LUMBAR SPINE WITHOUT CONTRAST dated 3/21/2008

TECHNIQUE
Multiplanar multisequence images were obtained without and with contrast.

FINDINGS
The L5-S1 level is assigned to the most inferior fully formed disc space and 5 lumbar type vertebrae are
assumed for numbering purposes on this exam. The conus medularis terminates at the L1 level and appears
normal. Their posterior spinal fusion rods of the L5-S1 level with localized metal artifact
There is no compression fracture.
Spondylolisthesis: Grade 1 anterolisthesis of L4 relative to L5, an interval change. Unroofing of disc material. Slight retrolisthesis of L3 relative to L4.
Curvature: normal.
Marrow: No suspicious marrow lesions.
Disc signal intensity: Mild disc desiccation of L3-4 and L4-5.
Disc space height: Moderate disc space narrowing of L5-S1.
Pre/paravertebral regions: No masses.
Conus medullaris terminates at L1. Conus appearance normal with no abnormal enhancement.
Miscellaneous findings: Bilateral small renal cysts. Posterior spinal fusion again seen at L5-S1.

T12-L1: No disc herniation, central canal stenosis, or foraminal stenosis.
L1-2: No disc herniation, central canal stenosis, or foraminal stenosis.
L2-3: No disc herniation, central canal stenosis, or foraminal stenosis.
L3-4: Slight retrolisthesis of L3 relative to L4. Small disc bulge. Ligament and facet prominence. Mild canal narrowing. Moderate bilateral neural foraminal narrowing.
L4-5: Grade 1 anterolisthesis of L4 relative to L5 with unroofing of disc material. Ligament and facet prominence. Mild canal narrowing. Mild bilateral neural foraminal narrowing.
L5-S1: No disc herniation, central canal stenosis, or foraminal stenosis.
