My wife feels like her insides are going to drop out of her vagina.

She was diagnosed with right sided Metastatic carcinoma on 5/5/2016. She opted to have a double mastectomy which was done on 9/13/2016. Had complications due to an infection on the right side. Surgery was done again 10 days later to clean up infection site, etc.. No further problems at that time except she has developed lymphedema to her right arm which we are dealing with. At least, as well as one can deal with it.

On 10/22/2018 she had Bilateral Delayed Breast Reconstruction with Deep Inferior Epigastric Peroration. All her incisions (each breast has a football shaped incision, abdominal incision runs from hip point to hip point) have finally healed on the outside. But she doesn't feel like her abdominal incision has healed on the inside. She feels as though her "insides are dropping out of her vagina". All the 911 calls I've been called out on (approx. 15,000) and I have no memory of ever responding to this specific complaint. I have seen this problem, although it was caused by auto vs pedestrian traffic accident and a fall from a rooftop onto a fence. Both did not have good outcomes, both died due to their injuries.

Is this possible? Our surgeon has been very evasive with his answers to our questions. Should we insist on an MRI, x-rays, etc? I'm usually pretty good at knowing what to do, and who to go to in most all sudden onset situations. Except this one.

Welcome to MedHelp and thank you for your question.  We appreciate your full detailed question as well.  For those reading, here is some information on the DIEP flap reconstruction after mastectomy https://www.webmd.com/breast-cancer/breast-reconstruction-without-implants#1 .  

My initial thought was perhaps a hernia, however, that is rare as the muscle is not touched during the flap surgery.  However, there are some cases of this reported although they are few.  So, that would be something to have checked.  Here is an article that has risks of this surgery https://www.mayoclinic.org/tests-procedures/breast-reconstruction-flap/about/pac-20384937. Scroll to risks.  My suggestion is to follow up with the surgeon that performed the surgery and raise your wife's concerns. They are legitimate.  It may just be the healing process but it should be investigated as she certainly doesn't need any set backs after all she has been through.

Women do sometimes have that feeling 'of their insides falling out of their vagina' and it may be unrelated to the surgery.  Vaginal prolapse happens for a variety of reasons.https://www.emedicinehealth.com/vaginal_prolapse/article_em.htm Could something like this be going on unrelated to her surgery?

I guess there could be lots of things going on that are unrelated to the surgery. After reading the above article, ie.. Vaginal Prolapse, that sounds like the most likely. I thank you for your quick response to this most concerning problem. Our next appt with the surgeon is next Thursday. For now we will have her continue resting as much as she can stand. Wow, that's kind of an oxymoron.  We will investigate further. Again, thank you for sharing your knowledge with us.              Matt
