White to yellow colored bump developed on nipple in last few months.

My dermatologist examined it and told me to put warm compresses on it.  Compresses seem to make it more irritated.  If it doesn’t help then she will drain/lance it.  She is giving it a month to see if it improves.  Is that the normal course of treatment and what may this be caused by - I had a breast ultrasound and could the probe not have been properly sanitized?
