Anyone have experience with Dr. DiNapoli at the Mayfield Clinic in Cincinnati?

I have an appointment for a cine MRI and a consultation with his PA at the end of the month.  First NS I saw recommended surgery based solely on size (15 mm)...not much discussion about symptoms.

Hi and welcome to the Chiari forum.

Did the first NS also check your CSF flow? If not, how would he/she know if surgery would be beneficial?

The first NS said nothing about CSF flow.  His explanation was: "If it's 0-5, we do nothing; 5-10, we monitor; over 10, it's surgery.  You are at 15.  When do you want to do the surgery?"  I'd already read enough to know that size isn't the most important criteria, so I am going to Mayfield for a second opinion.
I am glad to hear you are seeing more then one NS and getting more answers.....it just doesn't make sense that they (Drs) aren't aware it is not the length, but more so the width of the herniation and if there are other issues like a retroflexed odontoid or basilar invagnation.....sigh.

At 0mm's you can still have an obstruction - a gum drop can block things up more then shoestring licorice....no matter hoow long it is the licorice is only so wide, but a gum drop is just that a drop, a cork...and it can block almost all fluid if left untreated and cause many more symptoms then the long licorice.
