Five year old has trouble at school learning and focusing

I need help focussing my child.  He is 5 years old and in junior kindergarten.  His latest report card says the following:  " that he usually needs clarification to follow instructions and often needs direct supervision to get started at assignments.  He is often silly and off task in larger groups and needs many reminders not to distract himself and others around him.  When playing with his friends, he needs reminders to play cooperatively with others.  He can write his name but not identify the letters.  He cannot identify all the letters in the alphabet."

We just received my son's report card on Thursday, so I have not spoken with the teacher.

What suggestions do you have?  I need to practice his letters with him which I will do daily but what about the focus problems?  How do I teach him to listen to instruction and not distract himself or others?

Start him on Supplements a mutiple vitamin-mineral plus antoxidants Vit C and Vit B Complex, also some parents find fish oil useful, there is also a Homeopatic med called Attend by Vaxa getting good reports.
