Impulsive Child

I posted this in the ADHD forum but someone recommended for me to post it here as well..

My son is 4 years old.  He is a smart loving boy with an active, outgoing, fun loving attitude who loves to touch everything and ask questions.  

He has an issue keeping his handsHand or foot spasms
Hand tremor to himself.  In the past month he has hit classmates and had an issue where the teach asked him to put a toy away he held his handsHand or foot spasms
Hand tremor in tight fists as he threw the toy on the ground.  The teacher and myself are concerned for him and the other students.

We had him evaluated over the spring and the therapist said he was borderlineBorderline personality disorder ADHD.  When I mentioned that to the teacher she said he sits down and does his work very well.  Its just the handsHand or foot spasms

Pancreatic islet cell tumor she is concerned about.

I asked him why he got mad and threw the toy and he said it was because he didnt want to go to time out.  My guess is he feels that if the teacher asks him to do something it always results in time out thus the angerIslets of langerhans

With him being borderline
ADHD the discussion of meds has come up but I would rather try to solve this with behavior training because medication does train the right behavior but it might limit the reaction.

Any thoughts would be helpful

His anger takes the form of making very tight fists and holding them at his sides and crying.
He did not have these issues this summer.  In his new class with a new teacher he is having these issues.  We are considering moving him to another class as it might be how his new teacher deals with our son.( constantly getting on to him if he doesnt do what she asks right away.  We have found counting to 5 or giving him 2 warnings then timeout works best).

Yes we make sure he gets plenty of activity and sleep.

One more thing, as every fun loving, active 4 year old needs-----  make sure he gets tons of physical activity.  This directly helps behavior.  Don't let his teacher take away outside playground time as punishment.  Good luck.

Hi,  I'm trying to understand your post.  Anger, frustration and impulsivity are normal in a 4 year old.  They are still learning how to communicate when they feel this way and how to cope, so they have outbursts.  Diagnosing a child as young as 4 with ADD/ADHD would probably not be a good idea as kids vary a lot at that age and things that go along with ADD are normal 4 year old behaviors.  For example, it is 5 minutes that a child of 4 should be able to sit and do a project.  That is why preschools with the exception of open play have everything in super short increments.  Throwing a toy when angry is not unheard of in a 4 year old.  I would work to give him other ways of expressing it.  Help him use his words instead.  Model the behavior you would like to see when frustration/anger is happening.  Act the way you want him to act and even role play it out with him.  
Are you saying he has a hand tremor and foot tremor?  I wasn't sure what you were speaking of and that is something I'd see a neurologist for or at least his doctor.
I also didn't know what you were talking about ovarian cancer cells (is this a boy?) and pancreatic islet cell tumor.  But in terms of his behavior, I may have missed something but he doesn't sound to abnormal.  Good luck.
