Out of control 3 year old boy

My son, has taken me to the point where I want to pull all my hair out. He is out of control and there is nothing that works. He Hits, screams, Bites. Punches himself in the head, headbutts the wall and other stuff like the stove. Slams doors over and over and over until I remove him. No or stop does not work, taking things away does not work. Our dog is tied outside night and day because he beats on the dog. He destroys the house, writes on the walls, Will not listen to anything. Refuses to use his potty, poops on the kitchen floor. Throws toys at my face and other people. Will not play with other kids or talk to other kids. Refuses to go to sleep on his own. Hes up for 16-18 hours a day and will not take a nap. Will hardly eat anything all day, will not sit still for longer than 10 mintues. That sometimes is all in a 5 hour period.
On this good days he plays with his cars, or will draw pictures with me, those are the only 2 things he likes to do. He lines up all his cars, and markers in a row. Spins that wheels in his cars, and repeats himself over and over with the same works " Watch mom wheel spin". There is a family history of Biploar on my side. But my son was also born with Sleep Apnea, and would stop breathing 72 times an hour until he was placed on Pure caffine by his physician. This all has been a battle since he was 2 years old.....Please Help....I have taken him to his physician and she said its normal....I do not see how this is normal when my oldest did nothing like this. I need some advise please.

It would appear that his behavior is directly related to his lack of sleep. A severely sleep-deprived adult would exhibit abnormal behavior as well, though probably without the violence.

He is also no longer on the pure caffine any longer and has been off of it since he was 7 months old
