My son has uncontrollable itching!

My 4 yr old started itching 7 months ago. It started in between his toes and then his penis. Every couple weeks he has a new itching spot. Between his fingers, his gums, behind his knees, the corners of his mouth. Most come and go but the worst is his penis. That never goes away and it's horrible. He doesn't scratch he pulls it and says it itches on the inside.
We have seen every possible specialist and every med and cream possible! Urology, neurology,allergist,dermatologist, holistic... Nobody can help us! I'm desperate! This poor baby can't sleep at night! (Itching is always worse at night)
Please help!
