i need your advice urgently one week before my blood test done, test showing ALT 55.4 (testing level 9-50), and ultra sound report showing: fatty Infiltration of LIver. & BP 151/99. Is this going big problem for me. i m very worry and not sleep well after i see my test report results. Is this reversible? Are these symptoms of cirrhosis, HEP A, B or C. Whats should i do now. please advice

if ct scan excludes cirrhosis, then can we say that maximum my liver is at f3? if so, is f3 reversible?

Yes, if a scan is normal it would mean that worse case you would have advanced fibrosis (F3). When cirrhosis occurs then and only then does the anatomy/physical structure of the liver and the surround blood supply change abnormally. The liver itself becomes nodular. The size of different sections of the liver change. The surrounding blood supply around the liver changes with the back-flow of pressure caused by the scaring this increases vein and artery sizes. The spleen enlarges. All of these changes are obvious on any scan.

All stages of liver disease are reversal up until the liver decompensates with advanced cirrhosis. So even early compensated cirrhosis is reversible. It is only when the liver becomes decompensated and can no longer perform its many vital functions that the liver will go on to fail. For those with irreversible cirrhosis they will need a liver transplant to continue living as their own liver and its disease has gone too far to ever recover and function normally again.

So until a person has advanced cirrhosis their liver is still able to perform all of its life sustaining functions. When the liver begins to fail then we see the consequences of liver failure the ascites, varices, hepatic encephalopathy, jaundice, kidney failure, etc. etc.

Hopefully you will never develop cirrhosis. Fatty liver disease (NAFLD) is now the fastest rising cause of liver disease in the US so much research is being done to improve our understanding of it and improve its management. I hope you benefit from the latest developments.

I had non-alcholic  fatty liver 3.5 years ago.
At that time my ast and alt were around 200.
Then, I lost weight and then it is last 2 years my liver enzymes are normal.
My ct scan 6 months ago was normal. However, I have  bloating, gas, ringing ears and rumbling bowels.  Can these be symptoms of advanced liver fibrosis or cirrhosis?

Bloating, gas, ringing ears and rumbling bowels are not symptoms of cirrhosis.

Having a liver disease (such as NAFLD, hepatitis C, or any other liver disease) and having developed stage 4 (cirrhosis) damage to the liver are two very different things. People can have a liver disease and have very little damage to their liver (stage 0 or stage 1) or have advanced liver disease such as stage 3 or stage 4. The health of these people varies widely. From healthy to having a life-threatening illness.

The stage of liver disease is the measure of the amount of damage already done to the liver over time. Usually over decades. It typically takes 20 -40 years of chronic injury to the liver before a person develops cirrhosis of the liver. It doesn't happen overnight or over a year.

Cirrhosis is the last stage of liver disease that is why it is stage 4 out of 5 stages of disease. When the liver tissue has become mainly scar tissue which can no longer function properly and which can lead to liver failure over time if the disease process is not stopped.

No one with cirrhosis can have a normal CT of the abdomen.

AST and ALT are not indicators of the degree of liver damage, they only show ongoing current damage being done to the liver not damage to the function of the liver. They are given off into the blood when the liver cells are being injured. Because you modified your diet and lost weight if appears that the damage to your liver has been reduced, which is a good thing.

Bilirubin is one lab test that indicates liver function. It is the measure of the substance that causes people with jaundice skin and eye to turn a yellow brown color.

Best to talk to your doctor so you can better understand your fatty liver disease and what you can do to best manage it.

Have you had your bilirubin tested?
Jaundice, also referred to as icterus, is the yellow staining of the skin and sclerae (the whites of the eyes) by abnormally high blood levels of the bile pigment, bilirubin. The yellowing extends to other tissues and body fluids and also may turn the urine dark. Yellowing of only the skin also can be caused by eating too many carrots or drinking too much carrot juice.

Is this jaundice? My eyes are normal color. No.
Can you have jaundice only at feet? No.

Talk with your doctor to learn what stage of fatty liver disease you have so you are not worrying about complications of cirrhosis if you don't have cirrhosis. Some people with liver disease have little damage to their livers others of advanced serious liver disease.  Stage 0, 1, 2, 3 ,4 (Cirrhosis)..

Only people with advanced liver disease (cirrhosis) whose livers are failing are no longer able to properly process bilirubin develop jaundice (yellowing of the tissues) as a complication of advanced cirrhosis. These people should be under the care of a liver transplant center so they can manage their liver disease and be listed for a liver transplant..

Bilirubin is the substance that can build up in the blood and cause the skin and sclera (whites of the eye) and deeper tissues like the mucus membrane of the mouth to turn a yellowish brown. It is the substance that makes our feces brown during the digestive process. So people with jaundice can have pale or white colored stools when the bilirubin is not processed correctly.

Of course there are other medical conditions that can also cause high bilirubin levels. For example gallstone blockage.

If you are concerned see your doctor.
