Swollen lymph nodes in neck. Can someone explain this

Multilevel cervical chain lymph nodes as follows:

Right-side:
Level 1b: 1.2 x 0.6 x 0.8 cm.
Level 2b: 1.5 x 0.6 x 1.3 centimeters, 1 0.2 x 0.5 x 1.2 cm.
Level 3: 0.7 x 0.2 x 0.6 cm.
Level 5A: 0.8 x 0.2 x 0.7 cm.
Left side:
Level 1b: 1.2 x 0.6 x 0.8 cm.
Level 2b: 1.3 x 0.5 x 0.9 centimeters, 1 0.9 x 0.5 x 1.2 cm, 0.7 x
0.4 x 0.7 cm.
Level 3: 1.0 x 0.5 x 0.7 cm
thyroid appears to contain a solid, hypoechoic nodule within the
right thyroid lobe measuring up to 5 mm compatible with a TIRADS
classification 4 nodule
