please help

I am 47, have had 25 heart caths since nov 06,three heart attacks, three stents in the LAD,have three leaky valves,small artery dz, chf, and coronary art.dz. I have extreme chest pain and have spells of blacking out. they have done the halters, all that jazz, and now when i cant take it any more and go to to the Dr. or the hospital, they check me and then come off with " well you know you have small artery dz". They have tried all sorts of meds with no success, at on point they even had me on 100 mg of moriphine sulfate twice a day for the pain. the pain is still as bad, but i cant live on the pain killers like that. they have tried the nitrates beta alfa and calcium channel blockers, and i currently were a nitro patch in addition to the imdur and norvasc and using the nitro spray. my wife and i both used to be medics and know this cant keep up, please any suggestions?

Hi,
     Small vessel disease is primarily due to blockage of vessels with fatty deposits or spontaneous narrowing of vessels due to emotional stress. In most cases the exact cause is also unknown.
But the treatment for this condition is mainly with ACE inhibitors, nitroglycerin, beta blockers, calcium channel inhibitors, statins and angiotensin 2 receptor blockers etc. Also, supplementation with coenzyme Q10 and L-arginine has shown improvement in this condition. Pain is due to inadequate supply of blood to the heart and that should be dealt with. Hope this helps. Best.

Hi,
     Inadequate blood supply to the heart can happen due to blockage of the arteries supplying the heart. This blockage can happen either due to fat deposits or due to spasm. For spasm nitrates can be used. For fat deposits or atherosclerosis statins, antiplatelet drugs and weight management with keep your cholesterol under control can help. Hope this helps. Best.

thank you both for the information, some of these treatments have never been mentioned to me. we have tried several of the medications, but not all.Dr. Veena,how might you suggest to try and deal with the inadequate supply of blood? Any advice would be greatly appreciated, again thank you both from both myself and my family, they are getting tired of seeing me in so much pain and most of the time unable to even enjoy family activities.
