Is anyone familiar with what my problem is by a glance?

I took a little time to try to get a decent picture of the tooth in question - it's been a hole that was left to decay for the greater part of my life for fear of dentists and such. Stupid idea, yes. But beyond that, I've recognized that I need medical attention for about a year now. But with no healthcare, no employment, and no one gracious enough or carrying enough will to help me, I've been kind of stuck. I'm still rather concerned about this issue as it's been quite a plague since it came about. My cavity eventually decayed all the way through to the gum and developed a small lump about a year ago, this lump has since grown and developed a small spot of it's own. This tooth produces rather rancid and disgusting salive, has neverending recurring abcesses, and bleeds daily. I'm in a rather horrible situation at this point that's causing daily suffering, but luckily, it is at least numb to pain by this point. Useful information would be that I was a frequent smoker for about a year and a half and just stopped this past August. I'm providing a picture and hoping someone can either at least confirm my worst fears or ease them. Either way, I'd like some answers. I'm providing the clearest picture I took.

Any input would be appreciated. I'd especially know what kind of treatment I may need to seek for this, the risk factor involved, what it might be, so on. I'm desperate for answers at this point.

Tooth #30 appears to be hopeless. Seeing an oral surgeon is advised.
