LAB SCREW UP?

The try-in denture fit fine ~ it was GREAT!
The finished denture came back and it fit fine BUT there was a very small gap, at the very back edge.
It was sent back to the Lab.  All they were suppose to do was correct the gap.
It came back a second time and it would not stay in at all.
The dentist put it in my mouth and it fell on my tongue.  IT WAS THAT LOSE!

Okay, anyone have ANY idea what the lab did wrong to screw it up so bad?

The lab should have to fix it. I would insist. Don't take no for an answer.

What you said about chair-side relining material is true. The critical issue is , chair-side relining may not improve retention. It depends on what causes the loss of retention.

Yes, it is a complete (full) upper denture.  Sorry for leaving that part out.
I had read something like this not that long ago but was not able to find any information on it when I searched again.  If I'm not mistaken it had something to do with "movement of the mold" during the heating process.    
My dentist said he would do an "in office" reline on the denture.
On a NEW never used denture?  This makes me nervous. I read the acrylic material they use for in-office relines isn't as high in quality as the acrylic material the lab uses and this type of reline can discolor or obtain a foul odor.
I strongly feel, if it's lab error, the lab is responsible for correcting it.

Assuming your problematic denture is complete (full) upper denture, based on the description.If the initially processed  denture was retentive, and there was a small gap in the posterior border , the lab repaired the gap with lab processing procedure, the repaired denture may undergo dimensional distortion during denture processing.
