How can I make sure I won't go into relapse again?

Hi,
I was on Lexapro for 1 1/2 years and relapsed.  Now I am on Zoloft 100 mgs and it seems to be working.  I am afraid of relapse again.  I have more anxiety than depression.  What steps should I take to fortify myself that I won't go through this again.

Also, don't you think that a doctor should switch patient's SSRI periodically to prevent poop out, because that is what happened to me.  I saw on MedHelp that a person suggested that. For those people who expect to take it indefinitely of course.   What a good idea

You probably know to be aware of the warning signs.  I like to journal everyday so that I can keep track of my habits and my mood and help me to notice when I may be slipping.
I am not a dr but I don't know about switching SSRIs too often.  I do think that if you have one that works than perhaps the dosage may need to be adjusted sometimes.  I just had to go up on my Zoloft recently in fact.  I was bottoming out on 100 mg and just a slight adjustment to 150 was enough to fix me right up.  I have to be careful too because too much Zoloft can send me into mania so if I start noticing signs of that then I have to lower my dose but that is just me.
