What's this on foreskin pic attached

I had this around the area few days after unprotected oral,
A month later it's back doesn't itch very slight irritation.

Most of our members will not look at pictures of genitals, for good reason.  Also, this is not a diagnostic forum.  You might have better luck if you try to describe the lesion more specifically.

Good point I can describe red inflammation slightly raised red shiny patch.
It has been pretty much in the same area. It was 1-5 days after oral, 2 week and now a month.

They last 2 days max and then it's hard to tell it was there.
Do you mean I should post again?
If you haven't had any discharge, it is likely just irritation, and may or may not be related to the sexual activity.  If it ends up itching, it might be a yeast infection, which you can eliminate with anti-fungal cream.  It certainly doesn't sound like an STD.
