need information about acanthosis nigricans

My son has developed patches of velvety brown-black skin in his neck, (looks like a dirty neck), and underarms,  i know is acanthosis because his cousin has the same condition. My son is severely autistic with many behavior issues and getting blood work impossible unless sedated. He is on the heavy side but not obese, is there anything that works with this condition, the patches are really ugly and they seem to be getting worse. Is this something that a dermatologist can treat, do we need to see an endocrinologist? is there any oral, topical medication for it?  i have read general information about it and they suggest diet change, well... that is an impossible task with my son who has a very restricted diet (by choice) and would only eat few things and if not given to him he gets agressive and has terrible meltdowns. I am dealing with a very low functioning, severely autistic, big boy (15 y/o, 6'2", 240 lbs) Any input will be appreciated.

Hello and welcome to the forum. Sorry for this concern with your son. I would have some blood work done if you can through his doctor. You want to check on diabetes. People who develop acanthosis nigricans often are obese or have diabetes or both.  So, it would be wise to investigate further. I understand it is complicated with your son's autism, but this would be worth knowing.   Since you do the shopping, you have some control over diet. Losing weight may be what is required of him although I realize that is hard. Hard for us all but particularly with your son. But worth tackling if it helps diabetes and this skin condition both.  https://www.mayoclinic.org/diseases-conditions/acanthosis-nigricans/symptoms-causes/syc-20368983 Will he cooperate at all with these things? Has your doctor given you any guidance?
