Oh no!! How irresponsible for someone to let a 4 week old puppy go!!! It should still be with it's mother.
To start, just try straight goat's milk. If it won't lap from a bowl, get a bottle or syringe (no needle) to feed it. You can try giving it a little sugar water or Kayro syrup, a little at a time on the tongue.
When I wean my pups, I then start adding baby rice cereal to the goat's milk and eventually start adding puréed dog kibbke with that. What Janis describes sounds ok but I would just start with the milk and see how that goes.
The pup should be defacating on it's own but you can try some stimulation like Mom would lucking the pups.
Get a few cotton balls, or a soft cloth, and put warm water on then and half squeeze it out. Gently rub the lower tummy and vulva area in kind of a circular motion. Pick the puppy up and do the same to the rectal area. This may help evacuation.
The puppy has to eat and drink. Make sure it is getting fluids.

Oh boy,no a 4 week old puppy should still be with the dam and nursing
perhaps the mother is refusing to nurse(?) OK so let's try to help this wee
one.
Gerber strained CHICKEN baby food ( human baby food) one can of
goats milk( unless you have access to real goat's milk(?) Gerber RICE
cereal.
You are going to use 1 teaspoon of baby chicken,1 teaspoon of dry rice
cereal and 2 to 3 TABLESPOONS of warm goat milk,this should be mixed
well and it should not be too thick,put this on a flat saucer( plate) so the
puppy can lick it.The goat milk can NOT be used cold from the fridge,
You can put the milk( by itself) in a cup,then set this in a small bowl with
hot water to warm the milk it won't take long to take the chill off.
IF the little one licks this up then you will need to give this to the puppy
every 4 hours,you can also give the puppy 1/8 cup of just the goat milk
warmed,after it eats the cereal/chicken mixture to try to keep liquids in
the pup so it stays hydrated,it needs fluids but more milk than water right
now..this is a young puppy so milk is it's fluids right now.
Be very careful and when you take it into your vet for more help BE SURE
TO ASK THE VET if any dogs or puppies have been in with Parvo...
this is a very dangerous time for a young unprotected puppy and it is
way too young for vaccinations...hoping the clinic has not had a recient
parvo case.
IT IS NOT EASY to keep a puppy going unless YOU feed it every few
hours(yes,round the clock it can not go all night or all day without food
or liquids) and there is no BM because there is nothing in it's tummy to
poo out.
