Nothing is working

Have an 18 year old cocker spaniel with hip dysplasia  and dementia. Neither of the two have given him much trouble until these past few months, everyday he just keeps declining. He needs help getting up, he is restless all the time. We know he is the end stage and we are hoping he will just pass peacely at home but in the mean time we are all losing sleep just as much as he is, we have tried melatonin and benadryl neither have helped. Really don't want to take him to the vet, as that would just cause him more distress. Any advise on how to get him some restful sleep.
