Can the blood be removed with a needle , from the blood bubble in dog s ear, or does it require surgery?

Vets are really great too, so maybe try one if that is possible.

A blood pocket on the ear flap, called an aural hematoma, will heal on its own as long as the dog leaves it alone and doesn't mess with it.  When aural hematomas heal on their own, however, it almost always leaves the ear flap shriveled and kind of deformed-looking.  The best option is to have the vet perform a minor surgery to remove the hematoma, this way the tissues can remain the way they are supposed to and the ear flap itself will not be compromised.

When surgery is performed, after the blood is removed, a series of sutures are used to tack down the area where the blood bubble was.  When this is done, the skin is sutured to the cartilage so as to remove the space where blood and serum collect to prevent the formation of another hematoma.
