Ear fungus pain--why is it getting worse instead of better?

Two weeks ago I had a headache and a slight earache, so I took a decongestant, figuring my sinuses were acting up. Within 2 days, the earache was quite bad, so I went to urgent care. The PA there said it looked like I had put something in my ear. It said it looked "almost" like a fungal infection, but gave me antibiotics and polymixin drops to wash whatever it was out, and hydrocodone for pain. This treatment only made it worse, because it WAS a fungus. Drying my outer ear with a Q tip, there was black ashy stuff on it. When the pain was unbearable the next day, I went in again. This time the MD who saw me used peroxide drops, waited, then had my ear flushed to try to remove the 1" black blob she described. It didn't come out, but this process made my ear feel amazingly better immediately. She made an appt for me to see an ENT to suction out the crud, and gave me acetic acid drops. By the time I saw the ENT two days later, my ear was feeing almost no pain. He suctioned out the stuff and said my eardrum liked ok, but inflamed. He said this was one of the worst infection he had seen. He filled my ear with a yellow gooey antifungal agent, which cuts out air and sound. He said to leave the goo in and see him in a week. He also had me continue with my course of antibiotic. For the next 4 days it was great! No pain. Thought I was healing, but then the the pain came back in full force. (Could the antibiotic cause that?) Went from 0-90 in 24 hours, so I went back to see the ENT a day early. After sucking the stuff out (ouch) he said that it was better, but since it was still inflamed, we needed to fill it with goo again and wait another week. The antibiotic course is over, so I thought I would feel better again. But in the 2 days since then, the pain has gone to 100% unbearable. I have to take either 2 aleve or the codine pills back to back, and they won't last the weekend. I'm online reading about people who have had permanent hearing loss. I am a music teacher! I feel like the ENT Dismissed the fact that it is getting worse, not better with this treatment. The Peroxide and acetic acid made the pain go away, before... Should I go back to that? I think something is wrong if I am in so much pain. I want another opinion, but it is a weekend. I wouldn't be so impatient, but I don't want to lose my hearing! I also have had a (mysteriously) paralyzed vocal cord for 3 years, and I wonder if somehow this is a chronic condition that has flared up in a new way?
