Ear tinnitus and pain problem

I have problem with my right ear.

The problem started when I injured eardrum with pen. Eardrum is self healed for 2-3 weeks. During that time i have constant hissing ringing tinnitus.
The symptoms that i have now are many and changes periodically.
1. Fullness (not all time but with intensive tinnitus)
2. Pain
3.When I turn head right and left for many times my right ear have whooshing sound during movements,
when I stop movements the whooshing sounds stop in right ear.
whooshing sounds during head movements happens just when i have feel of fullness, pressure and a little pain in right ear.
As fast I turn head as higher is whooshing sound.
The Whooshing sound does not happen when pressure, fullness, pain and tinnitus problem decreases.
As higher are problems in ear as higher is whooshing sound and I can get it with slower head movements  
To be more precis when i do not move head right-left I cant hear whooshing sound.
It happens just when turn head and increases volume when tinnitus, fullness... increase.

Now days I have even jaw pain, and tinnitus increases intensity when i jaw clench or move neck
its like somatic tinnitus

Another symptom is that when i scratch forehead i can hear it just with my right ear but not with left ear that is completely healthy. With my right ear i can hear hair touching with fingers more than left ear.

My right nose closes when i have problem with ear and I think it happens because of middle ear problems.

Now days i have more problems and new symptoms because some time i can hear heart beat from right ear. I think it is happening because of pressure in middle ear or even in inner ear.

I have even euastachian tube disfunction sometimes. I can detect it when open or close door in car.

My questions are: Does Tensor Tympani Muscle have any problem and causes hissing and roaring tinnitus in some cases because of muscle tension. And does it cause pressure fullness, ear pain, jaw pain?

If i have problem with my inner ear, why should have pain in jaw?
In my case jaw pain happens because of middle ear problems.

Another info is that if i clench jaw in my healthy left ear i can hear a little tinnitus that is same like the tinnitus in my right ear but it disappears when I stop clenching jaw in my left side.
