Hoarse voice all the time

So i think my voice has been like this for a few years, but in the past few months my voice has seemed to be scratchier!! Every time i meet someone new, they ask me if i have a sore throat, if i lost my voice, or something of that sort. My voice always sounds scratchy and sometimes it cracks when i talk. I cant talk quietly, and i cant really scream. Also, when i talk for a long time, it becomes difficult for me to speak. Does anyone know what the problem may be or what i could do about it? Thanks!
