Sinusitis for 3 months - first time with sinus infection

In January I had a lot of congestion on my right side and a severe headache.  I went to by GP and was given 10 days of an antibiotic, when it did not subside, she extended it another week.  Still no progress, so had a CT scan and was told there was complete opacification of right maxillary antrum with soft tissue density extending into nasal cavity and anterior ethmoid air cells. Also enlarged right maxillary antrum, possibly suggesting a soft tissue mass such as a polyp or papilloma.  Also persistent opacification of the right frontal sinus.  I went to an ENT doc and was put on 2 weeks of yet another antibiotic.  Still a lot of mucus and less persistent headache, but still a headache.  Throughout I have been doing the Netti Pot twice a day and using Flonase. First sinus infection and rarely have headaches.  Neither have I ever had a history of high blood pressure, which is now all over the place.   One day it was 165/106, the next morning it was back down to 110/73!  Is surgery to clear it out my best option?
