Weird Symtoms with Chronic Sinusitus

I am a 23 year old male that is 185lbs and 6'0 tall. I have always been active and in good shape. Other than having allergy shots for about six years I have never really been sick or been to the doctor much. That changed in November of 06 when I got a severe case of Mono caused by EBV. I missed a whole year of school and really havnt been the same since. Since my diagnosis I now have chronic sinusitus and have been treated on a few occasions with antibiotics. Leavaquin was used once but after I did some research I wont let my doctor use that medication anymore. This is where my question comes in. Shortly after I got diagnosed I noticed minor "jerk" tremors in my lower limbs and hands. The doctor attributed this to anxiety and the infection in my body. Now almost 2 years later I still have the following symptoms and really am worried.

1. Joint soreness in my wrists and ankles. (mucles are fine and I have no weakness at all). This has happened over the last couple months.
2. An uneasiness type feeling when I extend my hands out
3. Random jerks and tremors mostly in my hands and feet only when I am at rest or nearly asleep. They can be in any muscle it seems
4. Burning sensation in my fingers and feet(burning sensation)

I am very concerned with my symptoms. I am a big time worrier and think I have all the worst diseases possible. I guess my questions are could this be a severe reaction to the levaquin antibiotic (it was taken more than a year ago)? Or could this indeed go along with the infection from my sinuses? I really dont know what to do.

Yah it dosent seem serious but i do notice the twitches if I leave my limp at rest for extended periods of times. My left big toe seems the most noticeable but it seems to be random. Out of my sore ankles and wrists my right ankle seems to be the part i noticed first and it seemed to spread. It is like a constant dull ache. I can still be active and so far its not hindering that way but I feel like its progressing. Could be all in my head. Hopefully this relates to my sinus infection

   Re; # 4; That does sound like peripheral neuropathy. Jerks when nearly asleep are normal as the muscles relax.
   Sorry; A Dr. should comment on the rest.
