ED problems are getting worse and worse

Hello, to see if someone has something similar and can help me. I am 25 years old, with a very slim build and do very little exercise. For a few years, I suffer from erectile dysfunction and premature ejaculation in any situation (it doesn't matter if it's sex or masturbation). From time to time, I have nocturnal erections, but I don't know if it's something daily or not, because I only find out about it once every 6 months, maybe. Recently I had significant fatigue, sleepiness, muscle weakness, hand tremors... but I have greatly reduced my amount of added sugar in my diet and those things have largely disappeared. I was considering that it could be a low testosterone level, but I've been like this for about 2 months and if before I never had erections, now even less. The only real pleasure I have is during ejaculation, which isn't even that great considering that the penis is extremely flaccid. The erection, although I can't call it that, because that's a shame, it does have a certain angle, but the penis is completely soft. I have to squeeze from the pelvic floor to make it thicken a bit, but right after that it goes back down. The size is ridiculous, much less than it was before, although in the flaccid state it gives me the feeling that it is bigger and stiffer than it should be. What seems curious to me is that, after ejaculation, the little that thickens remains for too long, that is to say, that it is difficult for him to lower the little that has risen. I don't have pain or strange things, nor do I have a better erection in X position. Even though I changed my diet to eliminate added sugars, I still have some problems that I had before, such as frequent and heavy urination, loss of hair and body hair, or muscle spasms in various parts of the body. I also have a lot of itchy scalp, very dry facial skin and from time to time spontaneous itching in many different parts of the body. In fact, now if I put up with it a little longer I start to suffer incredible testicular pain. Another thing that strikes me is the small amount of sperm that I have and that comes out in each ejaculation, in addition to the low force with which it comes out taking into account the state of flaccidity of the penis.

I've tried Kegel exercises, both normal and reverse, a long time ago and all I've gotten is an increased urge to urinate. I spent 3 months increasing my physical activity, walking long distances almost every day. I recently changed my diet, plus I stopped watching porn and significantly reduced the number of weekly masturbations to 4 or 6 tops. I don't suffer from stress or anything similar and there is no issue with romantic relationships because I don't have a partner or have anything. Mind you, though, I've never experienced any improvement with anything I've done and these days erections are even more ridiculous, when a change in diet is supposed to bring at least a tiny bit of improvement.

Any suggestions?

I'd suggest seeing a doctor for a preliminary evaluation.  If the problems you are experiencing are related to blood flow or your prostate, nothing you try with regards to nutrition and exercise will fix the issue.  Since you are having the problem when you masturbate, I suspect the issue is related to a physical attribute rather than a psychological attribute, but only a physical examination can determine the cause.
