SLT with Pigment Dispersion Syndrome..Dr Hagan

I have been taking Latanaprost and then Monopost for the last five years for mildly elevated eye pressure due to Pigment Dispersion syndrome..I had visual field test and OCT recently all good in normal range .

..I went to an Opthalmologist for a consultation to get the SLT done but he refused to carry out the procedure because of the fear of IOP spike and told it would be risky because of my Pigment Dispersion Syndrome but I believe my PDS is table now as I had it for a few years...I felt the doctor is not that experienced and I searched the matter with Dr.Google and found out that it can be done in my case with low laser power selected with good results...Any Suggestions Dr. Hagan?

If your pressure is well controled and your visual field test and optic nerve cupping and OCT are stable no need to have SLT. If something comes up where you cannot put drops in or the PDS goes out of control it is possible to do SLT in PDS but the eye MD was correct in saying there is more of a chance of a post SLT pressure spike than the general case. That being said I have  several glaucoma patients that has SLT by my favorite glaucoma specialist and did well.

Thank you very much for your valuable answer ...I will do nothing then and keep taking the Monopost...You mentioned that some of your patients had SLT with good results..Any of them had PDS ?
Yes, we are talking about a relatively low number perhaps 8-10. Some did have spike of IOP but it was managed with drops. The more pigment in the angle the better SLT works. All eventually go a dop of IOP on the order of 6-8 points
Thank you doctor very valuable information about the Pigment in the angle ..God bless you
Thanks and best of luck
