My doctor thinks I have a hiatal hernia and acid reflux based on the symptoms I described.  I've quit exercising temporarily because it makes my stomach ache when I do so.  I usually would work out on my elliptical or stationary bike but after about 5 minutes I start getting a little pain.  Nothing really bad but I just thought it would be better if I took a break cause I don't want to irritate my stomach.  See if your symptoms worsen when doing the Pilates.  If they don't I don't think it could do any harm.  Just my opinion.

Hi...with a hiatal hernia or acid reflux tight clothing can aggravate the condition. So, if in doing the pilates u find an increase of symptoms u may want to stop for a while to verify it is the pilates affecting how u feel.

I read the info on my pilates video and it only says not to do them when you have your period...so not sure.
I can't see how it could make the hernia worse...my son has to take gym class with his...and the doctor said it was ok.
