Problems After Biliary Bypass

My family member had a Biliary bypass because of high bilirubin levels (over 20)

The patient is a woman, 74 years old.
After the surgery is treated with perfusion with sodium chloride (saline) solutions and antibiotics.

There are 4 days after the surgery and she is not allowed to drink water or any other liquid food. Is this normal?
The bilirubin is still high(12). She has swollen feet and constipation.

What do you suggest we do?

Thank you very much!
