Severe constipation

Hi everyone,

I am a 33 years old male and have been pretty constipated for the past 2 months now... stool just seems very hard to come out. When it does come out it looks very creamy, so NOT hard, which poses the question why is it so hard to go to the bathroom?! If it does come out it is really thin, as there was hardly any space to come from. Sometimes there is some slime around the feces. I feel very bloated and feel the need to go to the bathroom but once I sit down I can't pass it out... i just pass out a lot of gas. Also a few times there seemed to be some pre-semen coming from my penis... I assume it is because I am pushing too hard to go...

I don't really have any pain in my stomach, I do feel some irritation around around my prostate but nothing crazy, maybe a 2-3 out of 10 pain... and only maybe 1-2 every 2 weeks... so not sure if related, I also feel a small burning on my stomach, about 3 inches above my navel - again like a 2-3 out of 10 (pain level). I have been given Lactulose by my doctor and that seemed to help.... but very strange the feces seem to be very water soluble... in the sense that once the feces hit the water (in one piece) they disintegrate immediately... seems like a scince fiction thing.  As I was taking the Lactulose I also got some foam around the feces.... I assume it is from the Lactulose... Does anyone have anything similar to this?  I am seeing a GI in 2 days.

below is letter I am going to give my primary to have on my file regarding the waste of time I had with the specialist

"I would just like to express my disappointment for the services that were rendered to me today. I had an appointment with Dr Rydel Warren at 2.30pm, doctor showed up at 3pm. Entered with my file and listened to my symptoms. I realized that he was more reading through my file rather than actually listening to what I was saying.

He then asked if I was under stress. I said no. Asked me if I have any other issues besides the reason I am here, I said nothing that is not under supervision or has not already taken care of. He asked me who lives with me at my house. Replied my wife, mom and son – 3 years old. Asked if I get along with my wife – replied yes. I did not have any stress at home. Told me that my file is quite heavy and that for a 33 year old I have the file of a 70 years old. Told me that he is not concerned at all about my symptoms and that I do look however a little stressed.  At this point he gave me a stomach examination and asked me if I take any antidepressants or if I am depressed. I told him no, not depressed, not taking any antidepressants – I am actually being more active now than before and feeling great besides my stomach concern.

He mentioned that I already had a colonoscopy 2 years ago therefore he sees no reason why I should get another one again. Again mentioned that he has no concerns regarding my symptoms. He prescribed an over the counter medicine for my constipation  - ducolax - and mentioned that the product that was prescribed to me actually can cause bloating. I told him that I was bloated actually before that and that the product seemed to work. I am not sure he heard that or acknowledged.

He then asked me where I am from and asked if I know certain Romanian athletes. We discussed sports.

I am just frustrated that I feel I have wasted ½ day from missing work and 40 dollars for a consultation where I feel I was not taken seriously. I believe that a patient should be consulted on the symptoms he has at that moment and the doctor should give objective advice and show a certain amount of care about what is being discussed.  None existed in my consultation.


As I left the office I realized that, although he told me to take the Ducolax for 3 weeks he did not mention how many times a day, or how or anything… I am to say the least very disappointed."

I am very disappointing in my Gi doctor. The visit went pretty bad starting with the fact that he looked over my symptoms 2 years ago and probably thought I was crazy as he did not take me seriously and just suggested I go on a laxative for the next 3 weeks.  

My symptoms are still the same

Severe constipation that went away after i started taking the medicine... now after I stopped it came back again slowly... on top of that now I can see almost every time I have to go no.2 that there is a clear substance coming out of my penis... before or after my stool comes out OR sometimes before AND after defecating... so I can pee while I defecate and then slowly at the end something like pre-*** slips out... no pain or anything in that area... maybe a small acidic feeling sometimes in my penis but no burning sensations or anything crazy

On top of this, a rash appeared on my forearm, about 6 inches long, itchy and extending a little bit each day. This came up about 3 days ago.

stool still creamy and sometimes smells like dead animal... about 2 times a week

again, same, when not sticky, feces seem to disintegrate as they hit water - so they are hard as they hit the water and then they turn kind of cottony disintegrated...

It feels like the muscles simply don't work when I am trying to cut the stool... it feels like I have to go more but can't push it out.

Also when I pee most of the time it seems to be coming out in batches... like I can not do a full long pee, but go a longer one and then is followed by about 5 more little squeezes, then towards the last ones the pre-*** like substance comes out... a few drups, but enough to see that the texture is different, slimey.

Same feeling like I need to go to the bathroom, once I go, push out and must that comes out is gas...

My weight is same. My forearm rash is mostly on my right arm, left arm I can see just a little bit.

Thank you Boron. I am seeing my primary doctor tomorrow to report the waste of time I had with the specialist and see where we go from there... personally I would like to do a colonoscopy....

Please let me know your thoughts on the above.

2 other things that I had in my mind were also a colon cleansing... I have some DR Natura from year ago when I did it for a month... also was thinking maybe doing a colonic... where they actually fill you up with water and cleanse you from the inside...

Thank you Boron. The stool is sticky but not white, more of a light brown. It was white about 4 months ago for a few weeks but didn't make too much of it. What I also see now is that the color gets darker toward the end of the elimination. So if what comes out first is light brown, what comes toward the end is very dark. The slime was there before and after the Lactulose. For the last 4 days I feel like it does smell pretty offensive, like rotten or a dead animal...

Again, thank you for your support. I will see a GI tomorrow and my primary suggested a colonoscopy. Will keep you posted on what happens tomorrow.

Stool may be thin from something what narrows the rectum hollow (inflammed prostate maybe?) or anal canal (hemorrhoids).

Slime (mucus) - if it appeared before you've started with lactulose may be from hemorrhoids, intestinal parasites, IBS or Crohn's disease to name main reasons.

Does "creamy" stool means "fatty" stool? Fatty stool is whitish, has offensive odor, floats, foams and sticks to the toilet. Fatty stools are due to fat malabsorption from small intestinalal inflammation, or from (and in these cases you'd probably have obvious other symptoms) gallbladder, liver or pancreatic disease.

Burning three inches above the navel may be from inflammation of duodenum (Crohn's disease, celiac disease) or peptic duodenal ulcer.

What to do?
Doctor (primary or gastroenteroloigist or proctologist) can make a digital rectal examination to check what obstructs passage of the stool.

For the burning above navel you can try to avoid acidic foods like citruses, soda, alcohol, also vitamin C or aspirin and maybe try some antacid medication you can get without prescription. If this helps it speaks for duodenal or gastric ulcer (or only an inflammation due to increased gastric acid secretion).

If creamy stools are from fat malabsorption, then avoiding fats would help against bloating. Lactulose itself can cause bloating.
