enlarged spleen and elevated alt

My 15 year old son has been having bouts of vomiting along with upper right quadrant pain, he was sent for a ultrasound of his gallbladder.  the ultrasound did not show gallstones but did show an enlarged spleen(17-18 cm).  Blood work showed that his liver enzymes was in the 200's.  On one trip to the emergency room for severe pain in upper right quandrant, the er doc did a hida scan but it came back negative for gallbladder problems as well although the doctor was almost sure it was the problem.  Tested negative for all hepatitis and also negative for mono.  4 months later my son still has elevated liver enzymes and an MRI was performed today.  Was told that his liver was slightly enlarged and there was a spot on his spleen.  I have not received an official report of the mri but would like any information on what direction I should look for a diagnosis.  This has been 4 months of elevated liver enzymes over 200 and the doctor does not seem to be in a rush to diagnose my son.  Did not schedule next appt until May.  Also would like to know if my son's physical activities should be limited due to the enlarged spleen.  Gastroemtology dr does not seem to think he should be limited but while researching information, I have seem that physical activity should be limited

Sorry, should have given some background info.  This is all new to me and I just want to know what is wrong with my son.  He is 5'10 and weighs 260.  No history of liver problems in the family nor does he show any signs of jaundice.  He is seeing a gastroenterologist.  Was sent to a pediatric gastroenterologist, but we did not agree from the beginning.  He said Logan's spleen was enlarged due to his weight and also that he had fatty liver without any testing.  I ask a surgeon that I was seeing at the time and he said that the only way your organs would be large because of body size is if your like Andre the Giant and then of course your organs would be larger in size but no due to just weight.  The peds gastro also said to come back in a month and he would put my son on some medication for his liver(did not specify what).  Also wanted my son to come back in a month, during this time he was vomiting at least 2 times a week.  Then one day at school the nurse calls and says she thinks my son is having a gallbladder attack.  We take him to the emergency room where a hida scan was done, results normal for gallbladder and he was tested for hepatitis a, b,c and viral hepatitis also tested for mono and he was negative for all.  The er doc had pulled his ultrasound and said that she did not see fatty liver but like I said, I'm new to all this and I do not know if you can tell that from a ultrasound.  I am still unhappy with the gastro. that he is seeing now because I do not want to wait until May to get the results of the MRI.  Also do you think that I should stop my son from seeing his chiropractor until I get the results from the MRI.  The lady that was doing the MRI said she was not supposed to tell us anything but she did tell us that the liver was enlarged a little and there was a spot on his spleen.  She did say that the spot on his spleen may be a blood blister.  Not sure what that is or why it would be on his spleen but I'm kinda worried about him seeing the chiropractor today.  I appreciate any information anyone can give me.

Geeeesh, what a nightmare. When I first started reading I totally thought Mono but then you said he was negative. I would be very concerned about your sons unexplained elevated liver enzymes and the enlarged spleen and liver, but I'm just being a mom, too. Any history of family liver problems or does he show any jaundice? Of course the enlarged liver and spleen could be a domino reaction to another problem. I can't say if May is soon enough but you need to find the reason and I know I'd be waiting at the doctor's doorstep the next morning if it was my son.
