Dr. Jose or Dr. Sean - Please Help : Its making me scared to death...!!!!

I already asked this in help forum but still couldnt stop asking to you Dr. Jose or Dr. Sean:

I am from Gujarat, India. This episode of mistake and foolishness happened on last thursday i.e. 30th June, 2011. I had an encounter with a Commercial Sex Worker from Bombay. First I fingered her. Then we went on to have bath. After the bath I touched her clits with the unprotected tip of my penis. Then we went on to have protected vaginal sex. The first condom slipped so I changed and put another one. This act was her upon me. I didn't reach orgasm so I removed protection and she sucked me unprotected and I came a bit in her mouth. Then finally she gave me a hand job. Thereafter in about next three hours I sucked her breasts thrice and deep fingered her vagina during that time. I don't know her HIV and STD status. I am having mild headache, loss of appetite, indigestion, sweating, feeling restless and sleep-ness since the day I was with her. My questions are :

1. What are the chances of me being infected if she was HIV positive?
2. When earliest can i get tested? What test should I take for early results since waiting for 3 months would kill me.
3. What best I should do so that I dont infect my two kids and wife?
4. Taking a PCR - DNA Test today would be adequate and reliable to know my status?

Consider the Indian Scenario for my case since its different than the USA - UK or other countries I believe.

The last few days have been very scary and it looks like I am drifting towards death. Please help and give me correct guidance and information so that I can start medicines if required. Please help me doctor. It has been difficult time all through. Waiting eagerly for your reply DR.

Thank you very much for your post.  All in all I do not believe that you have put yourself at any significant risk of HIV infection after reading your description of the events because there was no unprotected penetration.  However you did put yourself at risk of other STDs due to the unprotected oral sex and the brief contact of your penis with her genitals, including Chlamydia and Gonorrhoea, which you can be tested for straight away.  There is also a possible risk of syphilis; for that you will need a blood test and you would need to wait at least 4 weeks.  I honestly do not think that you need to worry about HIV, but what I would recommend is to get tested for Syphylis at 4 weeks, and at the same time you could have a HIV Duo test (which includes the p24 antigen) for your peace of mind.  Obviously my advice is not to have unprotected sexual intercourse with your wife until all this is clear.  Having a PCR DNA now would be a total overreaction for such a low risk encounter and it is not considered a very reliable test anyway.  Honestly there is no point in worrying about HIV, becuase I do not believe that you put yourself at risk in the first place and this is even assuming that the lady was positive.  This would apply the same in India, UK and other countries.

Waiting for the 4th week to get tested for everything and then move ahead in life with better things...

Will get back on here once I get my test results.

No, the reason for my advice is mainly regarding the other STDs, and not HIV.
The PCR-DNA for HIV-1 has been superseded by the much more reliable test, PCR RNA for HIV-1 and HIV-2.  The reliablity of the PCR DNA after 7-10 days is about 94%, which is not considered to be satisfactory (hence me saying that it is not reliable in this case; it would be totally reliable after 4 weeks though, as I mentioned in that previous post); the PCR RNA for HIV-1 and 2 would have a reliability of around 99.98% already after 7 days.  Anyhow, all this is irrelevant in this case as there has been no real risk of HIV infection anyway, and there is no need to do these tests.

Hi Dr. Jose could you please clear up this issue for the readers since you are makeing a contradicting statment in other post. Here, you said a DNA test is not a realiable test. However, in this post

Thank you so much Dr. Jose. That gives a lot of peace of mind. Surely I will go on for getting other STD checks. But the main thing going on was HIV which has come to a rest for now. Will surely go for HIV DUO test after 4 weeks.

I will get back to you here if there are further developments.
