HIV Risk From Incident and Testing

Hi Doctors,
I would like to thank you in advance for the service which you provide.

I am a South African male who is circumcised and Living in South Africa, I am concerned with my possible exposure to HIV on (7/07/2013), I visited an 'upmarket' white commercial sex worker/escort, and this is what transpired:

I gave and received unprotected oral sex, I licked her labia and not the vaginal opening and i also fingered her vagina without any barrier/protection. After this I had condom protected sex with her for the first time and to the best of my knowledge the condom didn't break.

I am concerned about my second exposure, when I had condom protected sex with the woman in question, from behind and I am unsure as to whether or not the condom slipped off whilst I was inside of her (I have never had unprotected sex before, so I wouldn't really be able to tell the difference in feeling), but when I withdrew my penis, the condom remained inside of her, my penis was completely uncovered.

This Incident has given me extreme anxiety. I took an ELISA HIV DUO test (4th Generation) on the 15th of July for a baseline and it came back non-reactive. I took an HIV-1 PCR DNA test on the 22nd (15 days after my possible exposure) and it came back negative. As I understand it, the PCR DNA test only looks for HIV-1 and not HIV-2, now I am going out of my mind, thinking that I might have contracted HIV-2.

I am planning on taking another ELISA test at 4 weeks and again at the 13 week mark.

1.Was I at significant risk for HIV?
2. Is the PCR DNA test accurate at 15 Days?
3. When is the ELISA considered conclusive? (some say 28 days, others 13 weeks and others say that testing should continue for 6 months)

Here are your answers as follows:
1.Was I at significant risk for HIV?
NOT AT ALL, PROVIDED THAT THE CONDOM HAD REMAINED IN PLACE AND INTACT.  IF YOU ARE UNSURE ABOUT THIS, THEN THERE WOULD HAVE BEEN A  RISK IF THE CONDOM HAD SLIPPED DURING PENETRATION.  HOWEVER THE RISK TO YOU WOULD STILL BE CONSIDERED RELATIVELY LOW, EVEN IN THE EVENT THAT SHE WAS POSITIVE.  THE ESTIMATED RISK OF INSERTIVE ANAL INTERCOURSE WITHOUT PROTECTION WITH A HIV POSITIVE INDIVIDUAL IS 0.06%
2. Is the PCR DNA test accurate at 15 Days?
HIGHLY RELAIBLE, BUT NOT CONCLUSIVE.  THEREFORE HIGHLY REASSURING.
3. When is the ELISA considered conclusive?
A FOURTH GENERATION TEST (WHICH INCLUDES THE P24 ANTIGEN) IS CONSIDERED CONCLUSIVE AFTER 28 DAYS.  A THIRD GENERATION TEST (ANTIBODIES ONLY) IS FULLY CONCLUISVE AFTER 84 DAYS.  I WOULD NOT WORRY ABOUT HIV-2 AS IT IS EXTREMLY RARE AND MUCH MORE DIFFICULT TO TRANSMIT THAN HIV-1.

Good day Doctor,

I just received the results of my 4th Gen Elisa, which I took 29 days post exposure(This Monday), the result was negative/non-reactive!! I am still awaiting the results of my second PCR DNA test which was also done on monday. Can I consider myself as HIV Negative and move forward with my Life??

Your previous reply stated that 28 Days with the 4th Gen Elisa is considered conclusive.

Thank you for help, doctor. I guess I will have to wait for next week to find out  to take another 4th Gen Duo test. I will have to keep my mind busy 'til then.
