worried about exposure and advice

Goodmorning doctor.I would like for you to respond to my question and my situation.

Well a guy gave me unprotected oral and fingered my anus with some of his own precum.I don't know if the precum was a lot,
but he touched the head of his penis with his finger and moistened it with the precum.It was just one finger.He then inserted that finger into my anus.It wasn't for more than two minutes or so as it felt uncomfortable and I told him to stop.

We didn't do anything else,no anal sex and I didn't perform oral sex on him.But the thing I'm scared about is that 4 days ago,he told me he just tested positive for Hiv.When he tested in December,he was negative which means he became infected over the last three months or so.My exposure was about April I think.

Yesterday a doctor at a local clinic told me my exposure was a big risk and that I should have been placed on Pep but it is too late now.So based on my exposure,was I at a big risk and is testing a must?

My friend is trying to cope...

You are correct in saying that:-

1) anal fingering with pre-cum is a theoretical risk only and there are no documented cases

2) you will not acquire HIV through being sucked.

Testing is a fairly individual decision. I would be comfortable personally that the activity you have described would not put me, if I were you, at risk. However, its you we're dealing with, not me, so you need to make that choice.

Just wanted to let you know that I did a lab Elisa for Hiv and it came back negative.Based on my calculation,the time since my exposure has been about ten weeks.Just wanted to let you know.Take care,you are a darling doctor.

Hello Dr Cummings.Thank you for response.

Now I have read other posts regarding anal fingering with precum on this site and the invariable advice is that it is theoretical risk at best and that testing isn't really necessary.

Now the precum on my partner's finger was a tiny amount and the fingering wasn't long.Also what about receiving oral sex-i read on this site that it is not a risk for hiv..Is it really necessary to test?If I don't have to because of this exposure then I won't.

Thank you for the post.

Recent infection with HIV means that an individual's body fluids - blood, semen, vaginal fluids etc will be highly infectious because there will be lots of virus in the fluid.

After a few months the amount of virus will decline and the individual becomes less infectious. It seems that you met this chap at around the time he had become HIV positive or shortly after.

There is detectable virus in precum - but not as much as in semen and it also seems to behave slightly differently with less infectious activity.

You are obviously past the PEP period now as your doctor has said.

You can test for HIV using an HIV RNA PCR test from 7 to 10 days after the event and to follow that up with a 4th generation HIV DUO test at 28 days post event.

I think on balance you will be fine actually but clearly a very good idea to check.
