Risk assesment: Quick unprotetected v*** sex, following symptoms and 4 week negative

Hello Docs,

I am a healthy 30 year old male from a w-European country. I have had only one sex partner in my whole life until 5,5 weeks ago when I met a girl (21), not CSW, in a regular club in Prague (stats, 2007 <500 HIV infected women in Czech).
We had sex which started with 5-20s unprotected vaginal sex which after I regretted and used condom. The girl namely didn’t want to use condom and asked if I had deceases since I wanted to use a condom. She said she has none.

After this event I immediately at that night started to regret and thought I had catch HIV and couldn’t really sleep. Ever since that, the whole HIV started to haunt and stress me. However, it didn’t stress me nearly as much until 1-2 weeks later I noticed a tiny ulcer in my mouth (I have them occasionally if stress) which took more than average time to get rid of (~7days) with mouth wash. I rarely moth wash. So I get stressed and think about being infected.
Little bit later I notice that my existing feet fungus (athletes foot) is becoming worse, spreads faster. I reason that this happens because I had started to train every other day in the gym (very sweaty exercises) and sometime used same socks twice.  Now I start stress even more and start googling HIV symptoms etc and get worried and worried.  2 weeks ago I notice
small tiny groups of blisters (just like in dyshidrosis except they don’t itch, only burn little bit) in one of my palm and side of several fingers. I google and found out that feet fungus and stress cause these blister symptoms.
Now I start stressing even more, even so much that I spend my full work days at the office only searching this site and trying to find that I don’t have HIV. Anxiety grows so much that I become extremely depressed and barely can function. Don’t eat (start loosing weight), heart pounds. I feel just like sleeping. Mouth becomes tingling and dry(occasionally). Palms and feet feel warm.

I tested after 32d and got Negative.
How big is my risk?

Your risk for getting HIV from this exposure is minuscule and should not be a source of concern for you.   As you point out, HIV is uncommon in Czechoslovakia.  Furthermore, even in the unlikely circumstance that she had HID, the average risk from a single unprotected sexual encounter is one HIV infection per 1000 acts of intercourse.  Finally, you have had a negative HIV test at 4 and 1/2 weeks following your exposure which is negative.  At this time about 80% of HIV infections acquired 4 weeks earlier would be expected to give positive tests.  Thus your negative test makes a very rare event even less likely.  I am confident from your description and your test results that you did not get HIV from the exposure you describe.    to confirm this you may wish to get another HIV test at 8 weeks following the exposure.  I expect that this too will be negative.

As for your symptoms, you have alternate explanations and in each case stress can make them worse.  In addition, sometimes after an exposure that, in retrospect, one wishes they had not had, persons tend to examine themselves and be far more attuned to genital sensations than in periods when they are not concerned.  This in turns leads to noticing what turn out to be normal sensations that might have been not noticed or ignored at other times.  Perhaps this was a contributor to your situation.  

Thus to summarize, there is virtually no risk that you got HIV from the exposure you describe.  I hope my comments are helpful to you.  EWH

During the ARS, HIV blood tests are sometimes negative but become positve about a week or so after that.  You really have no reason for concern.  This ends this thread. There will be no further answers.  EWH

Hi,

One more concern: If one actually has symptoms caused by the HIV before and during when the HIV test is taken (as in my case 32d), is it higher likelyhood of getting a positive result or do they effect the results at all?

The ARS is totally non-specfic and your symptoms are nto suggive of the ARS.  Do not worry.  EWH

I am much less worried now due to your comments plus the negative results.
However, I am wondering regarding my symptoms. Are these common early HIV symptoms or is the so called ARS usually the only early symptoms?
