Starting a new relationship, want to move on with my life

Hello doctors, thanks for your time and all the good work you do here. I understand it can be extremely tiresome to do this but please be patient with me.
First I will explain my history:
late 2012 - march 2013, 5 encounters with csw, protected vaginal, unprotected oral, i recieved every time and twice i gave. Protected vaginal sex, no condom breakages that I am aware of, can't be certain about condom slippages.

mid july 2013: unprotected oral with csw, protected oral.

had a INSTI rapid blood test 5 days after the latest encounter(ie. in mid to late july),  - it came negative.
had STD tests for all other STDs in mid august, all negative.

Additional information: I have a pilonidal cyst(an abcess in my cocyxx http://en.wikipedia.org/wiki/Pilonidal_cyst) and it occasionally bursts without me having any idea. I am wondering if this is a potential pathway for infection.

So my questions are:
1. Would you recomend additional testing? The clinic I tested at said that my Insti test did not cover my latest encounter.
2. Can I have unprotected sex with my new partner?

Thanks a lot for your time and God Bless.

Welcome to the Forum.  I will try to help.  First of all, let me remind you that most commercial sex workers do not have HIV.  Even if one of your partners did, most of the exposures you describe have no meaningful risk for HIV associated with them.  Condom protected sex is safe sex and there are no proven cases in which a person has gotten HIV from receipt of unprotected oral sex from and infected partner.  There MAY be a few (less than five) cases in which HIV was acquired from performing unprotected sex on an infected sex partner but these are also exceedingly rare.

Your pilonidal cyst does not change your risk for HIV in any meaningful way, even if it was draining at the time of your exposures and even if it was contaminated with a partner's secretions during sex.  Cysts of this sort are not pathways for infection.

Your negative test in July provides conclusive evidence that you were not infected during your encounters in March of 2013 or before that time and as I said above, your protected genital sex and receipt of unprotected oral sex is July were no risk events.  Therefore, I would not worry further and see no reason for further testing.

As for your specific questions:
1. Would you recomend additional testing? The clinic I tested at said that my Insti test did not cover my latest encounter.
It is true that your July test did not cover your encounter a week earlier but there was no meaningful risk associated with that exposure.  I would not test further if I were you.

2. Can I have unprotected sex with my new partner?
Yes, you can have sex with a new partner without fear that you might give them HIV from the encounters you have described.

I hope these comments are helpful.  EWH
