Worried SICK possible HIV infection

Doctors: I have read all of post, but I need some further advice based on my unique circumstances.

Last night, I met a guy (said he’s bi).  I always practice safesex – no anal sex, and rarely oral sex.  I told him that I wasn't interested in anything other than kissing and JO.  He said that’s all he wanted to do too.  He told me he was HIV neg (I don't know when he tested last), and that he always practiced safe sex.  We kissed and masturbated each other for about 10 mins.  At some point, I felt his penis rubbing up against the area between my testicles and anus.  I'm not sure if his penis actually touched my anus, but its possible.  I didn't like his penis close to my anus, so I moved it away.  I know for a fact that there was NO penetration.  My major concern here is that I have a very sensitive anus.  Whenever I use toilet paper, I see little blood stains.  I had used the restroom 1.5 hours before this and I noticed blood specs on the toilet paper.

Then, he rubbed some lubricant on my penis and on his.  We kept touching each other for a 2 mins. He then put his finer in my rectum.  I didn't see if he had any precum in his hands since they were wet with lube.  He fingered me for less than 5 second.  I am not worried about the masturbation.  However, I am concerned about his penis getting close to my anus (or touching it lightly?) and the fingering part, in light of the specific circumstances that I described concerning my blood spotting on the toilet paper.

Called two HIV hotlines and received conflicting advice - one said low risk; other said somewhat risky, and maybe a PEP.  

It's been a little less than 24 hrs, and I am considering going to the ER for a PEP.  Your guidance is appreciated. Am I at risk of HIV from (1) his penis getting close (or touching my annus) and (2) him fingering me.  I just couldn't tell is he had precum on his hands, but I did have blood spots on the paper when I used the restroom 1.5 hour before.  Should I get a PEP?

No, the presence of blood on the TP is not worrisome.  Even in the unlikely circumstance that you suggest did happen, surface contamination of a "wound" would not lead to infection.  For HIV to cause infection in a wound the material containing virus would need to be introduced deep into tissue, not just contaminate the surface.  EWH

Welcome to our Forum.  Decisions about HIV risk and the need for PEP and you will ultimately make this decision yourself.  

If you choose to pursue PEP, I am certain that you could find someone to provide it but if you came to see me, I would work to talk you out of it. There is nothing you describe that puts you are risk for acquisitoon of HIV.  That includes the fact that his penis was close to your anus, that your anus tends to be sensitve and sometimes you see blood on toilet paper and the fact that he insered his finger, which may have had genital secretions on it, into your anus.  these are all no risk exposures.  No penetration occurred.

Still no risk.

I would add, it sounds as though you have hemorrhoids as a cause of the blood you see.  You might want to get this checked.  EWH

Thanks, Doctor.  I promise this is my last post in this regard.  

Is it true that HIV tranmission in this case would not occur even if the "wound" caused by TP friction 1.5 hours before has not healed yet? The reason I ask is because last night, I used TP after using the restroom.  I waited 1.5 hr, and then checked to see if there was still an open wound.  Unfortunately, to my surprise, when I placed TP in the area 1.5 hours after, I still saw little specs of blood on TP.  I am concerned that when I had this encounter, this area still had open wounds that would have allowed the virus to go in.  Would this still be considered surface contamination?  

You have really helped me and calm my fears.  I just need some further clarification.

Thank you, Doctor.  I really appreciate your advice and your extremely quick response!  I haven't been able to concentrate or do any work today, and I am very anxious about this entire thing.  Just so I understand, you don't think there was risk of trasmission even if I saw blood on toilet before 1 hr and 30 minutes before this happened?  Wouldn't this be a place for HIV to get into the bloodstream?

I'm not trying to be difficult, I am just worried and I want to put this behind me.  Your reassurance and medical opinion is very important to me.
