Exposure after seeing an escort, concerned about adaquecy of testing. please help

Hey doc,
First off, especially given the corona situation, I just want to say that I have the utmost respect for health care professionals and salute you guys for your selflessness. And thanks for everyone taking part in this wonderful community.

A little bit about myself, I'm 29. When I was 20 to 25, I probably visited 4 to 5 escort/stripper (always used protection). And I never got tested. During 2019, I visited an escort on October 5. This was the only exposure I had during 2019. There was some light kissing and protected vaginal sex. No oral/anal. (Although she was about to give me unprotected oral and I stopped her. But I guess unprotected oral is prob a pattern of behavior for her). I'm not certain on what kind of condom is used, my guess is either latex or polyisoprene. I pray that it's not lambskin. We also used some lube. When I messaged her what lube she usually uses several months after the fact, she says she uses either silicone or water-based lube. The sex didn't last too long, maybe 5mins, all miss. style and very gentle. I remember pulling out and finishing outside with my hands but in the condom.

Anyways, I didn't think much of it until I went to my parents' house the following weekend. Their place was kind of cold and I developed a sniffles. Then I watched the movie Straight out of Compton and the Easy E scene scared me to my core. After some googling, I couldn't shake the fear of getting infected with hiv.

So on October 15, about 10 days after the exposure on Oct 5, I went and got a comprehensive STD screen test and everything was good. Specifically: Hep B - antigen non-reactive. Hep B antibody detected which suggest immunity (prob due to the vaccine I had in the past). Hep C antigen & antibody non-reactive. HIV P24 antigen/Hiv I & II antibody non reactive.

So far so good. A week after the test, however, my body broke out to hives for a few nights (they seem to disappear in the morning and come on at night) They were very itchy and freaked me out. Anyways, the hives went away on its own after like 4 nights. And then the week after, I started having very dull pain in what appear to be my kidney area. After about 2 weeks, the pain subsided. Not wanting the potential bad news to ruin Christmas, I didn't take any more test. During the holidays, I had a bit of a sore throat that is very mild and doesn't hurt when I swallow.

On January 4, 2020, (90 or 91 days after exposure) I went to Planned Parent and got the rapid HIV point of care test. After some research afterward, I believe it was the Insti test kid. The result was negative.

Afterwards, I have been having a white tongue to this date (March 30). In mid Jan, I did let a walk-in clinic doctor take a quick look at my tongue and he didn't see too concerned, saying it's prob a yeast overgrow and told me to just brush the tongue and return if it didn't improve in 2 weeks. However, I didn't get a chance to go back due to the coronavirus. And starting yesterday, I have been experiencing some tightness/pain in my back which is suggestive of potential lung congestion/issue.

A couple of questions I have:

1. If there's a minor tear/small hole in the condom, would I have been about to notice it if I didn't examine it too closely? I don't believe I noticed any liquid leaking out of it after I finished it in.

2. When I was taking the condom off, I had to kind of roll a portion off my shaft. To tie it after, I had to unroll it which meant I fidgeted with it for a while with my hands. Is there risk that there's sufficient fluid on the outside of the condom that touched my hand? The outside of the condom feels slippery but it could be due to the lube.

3. If my body produces antibody for Hep B, does it suggest that it should be able to product antibody for hiv no problem?

4. Could the liver area pain suggests co-infection with Hep C? Thereby delaying the hiv antibody protection to post 3 months?

5. Since my 3 month test was an Insti test, should I do another Gen 4 combo test to rule it out or is the Insti test sufficient?

Many thanks!

Whew, that is an epic post.  You had protected sex, correct? Protected sex is not a risk for HIV transmission.  Adults only get HIV from unprotected vaginal or anal sex or sharing IV drug needles. Air and saliva inactivate the virus.  So, it's not surprising that any tests you already have taken for HIV were negative.  I'd have expected them to be since you never had a risk. All of the info provided, although very thoughtfully given, does not make this in any way a risk.  So, answer to number 5, no.  You have no need to test.  Answer to number 1 is that this doesn't happen.  Condoms burst wide open and micro holes do not happen.  Your condoms were intact or you'd know.  Answer to number 2, only the head of your penis needs to be covered, so irrelevant. The other questions don't really make sense and hepatitis testing has nothing to do with HIV.

So, that's it.  Time to let the fear of HIV go.  You've done a good job using condoms. Continue doing that and you should never have to worry about HIV.
