HIV exposure & testing query

Hello, MedHelp experts,

I am in trouble for last 5 weeks. I am from Delhi, India. Around 5 weeks ago on 23rd Dec 2018, I went to a Delhi based massage parlour. The massage ended with she masturbating me and during the course, I sucked/bite/licked her breast and nipple for a brief time and but suddenly I tasted something sour/salted or strange thing on my tongue and immediately I tried to spit it out but nothing came out. After this I did not suck her breast and took the bath and left. I also enquired the masseuse if she is married to produce milk to which she denied. I trusted her and moved on. But as days passed I keep reminding me of the incident and started panicking and I took the "HIV Retroquick" Antibodies, serum test at two weeks after the incident and it was non-reactive.

After 3 weeks of the incident I travelled back to my home town(overnight journey in the train) and because of climate change in North India, I caught the cold and cough along with running nose. It lasted for a week. I panicked more and did more reading about the symptoms and was not able to be around my family and could not focus on work. As soon I return to Delhi I went back to the hospital for cold and informed the Doctor about the massage incident and Doctor asked me to go for a P24 antigen test.

I took the HIV 4th GEN ASSAY(P24AG + HIV AB), Method: CMIA post 29 days(21st Jan 2019) of massage incident. It was non-reactive for both antigen and antibody. The test was done at SRL Lab.

I have read from lots of thread on Medhelp that it was no risk event(no matter if milk/blood/pus or whatever came out from masseuse nipple). But still I am not able to keep my self focused and every now and then I googling stuff which is making more anxious.

It has never been with me, I freaked out so much that I went to the massage parlour again to enquire the lady about her HIV status to which she replied she never did HIV test. Just to add her both nipples were in different shape, one being upward and another kind of inverted and having little white dried stuff on it. I sucked/licked the inverted one. I thought I was being disrespectful to the lady asking her about her HIV status so I left the place with all the anxiety.

I would request you to please help me with the situation. Shall I go for another test at 6 weeks window? Please Help me.

1. I will repeat what every one has already explained to you precisely - Nothing that you mentioned puts you at a risk for HIV.  Kissing, sucking,  masturbation etc - None,  Zilch, Nada - No Risk.

2. You do not need any test to determine if you were infected from this incident, it is inevitable that you can't acquire HIV from any of it.  There is no scope of transmission.

3. Your speculation of acquiring the virus from ingesting breast milk containing HIV is not viable, it is only a 'theoretic' risk for infants born to HIV positive mother from drinking mother's breast milk, since their oral mucosal lining could be underdeveloped and that could cause the virus to contact their blood stream. Which certainly is not the case with you - You are an adult.

4.  Your fears are irrational and you have no HIV concern whatsoever, your questions no longer belongs to this forum as we have fully established that you are HIV negative and there was no risk associated with your incident.

5. You are either worried about an irrational fear or grieving passively about an underlying emotion which further could be guilt or obsession or something else, you need to work with a professional to determine the factual reason and right course of action. HIV is not your concern.  No testing necessary.

Also, you probably tasted a mix of sweat and dead skin remains that you spat out, you just needed some listerine and no HIV testing. Save your money, just my two cents.
Yes quite possible, I did not thought that way. I would not have worried much if I had not caught the major cold at three weeks mark and secondly seeing her nipple afterward which was not normal and I started thinking irrationally.

Also Lab did made some mistake in test code for cmia test and I got the result with different accession number with the one I was billed for. The series of events just started boiling up.

These are not risks for HIV. The only way those who get HIV is from unprotected vaginal or anal intercourse or sharing of IV drug needles. Saliva and air inactivate the virus. So, zero risk for HIV.

Hello @GuitarRox, thanks for your response and assurance. I am trying to keep the mind busy with productive things.

Since it was dark there in the massage room I could not see her nipple clearly that time. But later when I visited again to enquire I asked her about the HIV status and if she is lactating she showed me her nipple. Her inverted nipple was very strange as if it has badly bitten before as it was split into two part and some white stuff was accumulated in between them. It was the same nipple that I sucked.

I worried only after seeing that nipple as I never seen like one. I am worried if that nipple had a fresh wound or blood when I sucked it(the strange taste I felt). If that is going to make any difference?

Moreover, she was never tested for HIV and when I asked her to get checked she freaked out. I am sorry but this is how many people are here and remain ignorant about diseases.

However, I did "HIV Retroquick" Antibodies, serum test at 2 weeks and HIV 4th GEN ASSAY(P24AG + HIV AB), Method: CMIA post 29 days of massage incident for the peace if mind. Both were non-reactive.

Please confirm if sucking fresh wound or pus on a nipple is an HIV risk?

Already answered you fully.  Not a risk.
Sorry I am being very anxious.

I there is any remote change of HIV considering I had my 4th GEN ASSAY(P24AG + HIV AB) test which was non-reactive, Test Method: CMIA post 29 days of incident.

Assuming it was freshly wounded/pus/blood nipple that I sucked.

If any further testing required as I had a word with local HIV help line number and they suggested for 3 month antibodies test.

This answers all of your HIV questions, and if you can think of any more just reread about the 3. You had zero risk therefore  testing is irrelevant to your situation because you had zero risk. HIV is a fragile virus, which is instantly inactivated in air and also in saliva which means it is effectively dead so it can't infect from touching, external rubbing or oral activities. It doesn't matter if you and they were actively bleeding or had cuts at the time either because the HIV is effectively dead.  
Only 3 adult risks are the following:
1. unprotected penetrating vaginal with a penis
2. unprotected penetrating anal sex with a penis
3. sharing needles that you inject with.
The only way to get HIV is if you did one of the 3. The situation you describe is a long way from any of these 3.
Even with blood, lactation, cuts, rashes, burns, etc the air or the saliva does not allow inactivated virus to infect from touching, external rubbing or oral activities. Doctors have calculated the risk from what you describe to be less than that of being hit by a meteor, therefore no one will get HIV from what you did in the next 40 years of your life either. The above HIV science is 40 years old and very well established, so no detail that you can add to your encounter will change it from zero risk.
