HIV possible exposure

I have a girlfriend. We often having sex. I always use condom for penetrated her. In the middle of the intercourse, I always check the condom and condom always still intact. But my concern are,

1. sometimes during the intercourse, I change the condom because the condom is little bit loosen. So, of course her vaginal fluid touch my penis when I change the condom. Am I at risk?

2. Last time I having sex with her, I stop in the middle of the intercourse to check if the condom still intact or not(as usual) after he orgasm. But stupidly I draw the condom little bit forward to make sure the condom cannot be slipped and always cover my penishead during the intercourse. And yes it cannot be slipped. But when I draw it, there is some orgasm fluid in the condom ring that touch my foreskin. After that, I draw the condom backward to cover my penis, and carry on with the same condom. And of course there is some orgasm fluid inside the condom. Am I at risk?

3. One more question. Is it just the same between vaginal fluid and orgasm fluid? I heard that vaginal fluid isn't infectious, if it is true, so the orgasm fluid isn't infectious, right? And where are those two fluids come from?

thank you all. Now I feel much better. But I'm still wonder. Is it just the same between vaginal fluid and orgasm fluid?
Or there is no orgasm fluid?
I mean when a girl orgasm, she produces the same fluid like when she excited?

Her having HIV does not change the answer.

Yes, the fluid a woman produces when aroused is NOT infectious

i'm not sure. But she told me that she was almost never uses condom while having sex with people before she had relationship with me. If even she have HIV, I'm still love her and I will always use condom for intercourse.

If she hiv+, will it change your answer?

And what about answer number 3?
