Please help. Really Confused. Desperate for help !

Hi,

First of all my sincere apologies for this long message. It was not intentional. But based on my country of residence, I hardly have any knowledgeable people to talk about this. I ask for your apologies on this.

I am from a South Asian country. On 10th June 2014 I was given unprotected ORAL SEX (Blow Job) by a Russian sex worker. No Kiss of any sort involved there. No fingering. No tight hugs even. She put her tongue into my anus 4 times (for about 5 seconds) and then all there was oral sex she gave to me (Her mouth to my genital). To be honest, it was my FIRST EVER encounter with a woman. I have never ever had any kind of sexual encounters earlier either with males or females. I am 33 years.

Scene
The oral sex lasted for about 40 minutes. First for about 15 minutes she gave me oral and I ejaculated into her mouth. Then again, she gave me oral for about 25 minutes and then I ejaculated into her mouth. After this, I got scared badly. Then Googled and found out oral sex was less riskier. I contacted her and she sent me a text message "I don't have any STDs if you meant so. I have medical reports I'll show you next time" - however, I was not fully convinced to trust her! Within 72 hours time, I visited an specialist physician (Not an STD specialist) and he told me "If you only engaged in oral sex, you have no risk, even if she was HIV Positive". Further he prescribed me Aciclovir and Doxycycline to prevent any possible Herpes, Syphilis and Chlamydia infections. I was satisfied with his approach and they were similar to what I read on internet. I came out happily.

Issue
This was my first (and upto now the last) sexual encounter with a person. I have not married yet and had no sex earlier with others. While my days were passing without any kind of problems, on the 25th day after the oral sex exposure, I felt something was not well in my body. But I did not care. On the 26th day I felt something like a golf ball stuck at the lower-end of my throat. Due to this, I felt unwell and slowly & slowly anxiety and fear started to grow on me.

27th Day: I went to a non-specialist doctor. He told it might be GERD problem despite I told him I feel no acidity in my throat. Gave some pills.

28th day: Feeling in throat reduced but was still feeling unwell. Not much appetite.

29th day: Morning - Extreme vertigo. Reduced after having some lime juice along with his pills. Evening - Unfortunately, the body temperature started to increase. I was totally down that I might have infected with HIV. Went to Dr again. He did not measure the temperature, but placed his hands on me and said it's my body temperature. Gave paracetamol.

30th Day: Temperature is not high. Taking Paracetamol. No vertigo. Saw tongue in the mirror and found out it's "White-coated". Unable to taste foods much. Further, I saw red-throat (Painless sore throat).

31st Day: Though I felt like I have low-grade fever, my digital thermometer shows temperature in the range of 97.5F - 99.32F. Evening time I was NOT feeling elevated temperature. (So effectively this elevated temperature feeling was there for about 48 hours). But was on paracetamol. Sore throat continues.

32nd Day: Things get reduced. Temperature is within normal range. Not much appetite though. Still mild feeling that something stuck in my throat. Sore throat remains the same.

33rd day after exposure (Almost 5 weeks and around 7 days after the onset of these issues): Went to a lab and did a quick test. They said it's Strip test, but was unable to comment further about the test generation. HIV Antibody 1 & 2 test came NEGATIVE. Then I asked for an ELISA using the SAME SERUM collected. They said it's 2nd Generation they use although they are an ISO Certified lab. After 2 days results came as NEGATIVE.

34th day and onwards: Things became fine, Went to my specialist Dr and he ruled out HIV thing and prescribed me Augmentin antibiotic. But Sore throat did not go away. He asked from me "With having a 33 day HIV Negative report, why man you're scared ?" :-/

But it took me about 11-12 days for the "feeling something stuck on my throat" to go away. About 7-8 days for the white tongue to go away. But just about 2 days for vertigo and elevated temperature to go away. AND - Sore Throat (Red throat. No Pain at all!) is still there. Say for more than three WEEKS! and counting.

These days I was and still am scared to death thinking whether I had infected with HIV. At times I get afraid thinking whether these are ARS symptoms. But at times I think these might not be ARS, as I have read in medical websites saying "ARS symptoms come together and leave together". But in my case, the sore throat is still there for 3+ weeks, while other issues had diminished about 2+ weeks before. I did NOT have rash or swollen lymph nodes (lymph nodes were examined by 2 doctors and confirmed they are normal).

At the moment, I am really confused and don't know what is going on. Specially, I got these symptoms within the 2-4 week time limit and I could find NO BASE for these illnesses except the long 40 minute oral sex I was given by that CSW. At times I think, whether she was recently infected and her saliva might have contained enough viral load to infect me ?? Because 40 minutes mean not a brief BJ session! I'm aware saliva is not infectious though. I could NOT see any blood during oral sex, but as like others, can not be 100% sure. I am sorry for my long message. Please enlighten me on this. Is this situation related to HIV ? What would be your views of a 33 day negative report (ELISA 2nd Gen) considering this is Insertive oral sex, BUT with some health problems after 3 weeks ? Do I need further testing for HIV ? I thank you for your time to read this message.

Hi,

Update:

Visited ENT surgeon and he told it's silent acid reflux (LPR) by inspecting the throat.

Further, also had two HIV tests (Strip test - should be 1st or 2nd Gen) and ELISA test (2nd Gen) after 62 days (almost 9 weeks) and it's NEGATIVE !

I believe things are fine now. What you think ? Thank you my dear :-)

Thanks.

I really don't need a conclusive result now. But just wanted some assurance. As per Dr HHH and Dr Hook, when the exposure is low risk, even a 85-90% assurance is considered conclusive. I also saw that they have said in about 28 days, detectable antibodies might have formed if a person is infected with HIV.

When I last visited my Dr, he asked from me whether I have any damaged teeth. I casually said "No". But just after 2 days, I saw one of my teeth been damaged. Already it had been filled by a dentist, but the filling came out from the teeth. In another medical forum an MD was saying: Q: Can bad teeth cause you to have a sore throat? ... Dr. Joel Doyon answered: Possible. A sore throat is usually a result of bacterial or viral infection. It's possible that a 'bad' tooth has an infection at the base of the root and gets into the bloodstream and then settles in the throat. I would recommend seeing a dentist to get your teeth evaluated.

Rather a dentist, I have booked an appointment with an ENT Specialist this evening to discuss about my throat.

Antibody only test is conclusive at 3 months. I took a flight to get my test and peace of mind. It was worth it. Surely, will not go to csw rest of my life.

Thanks for you too for comforting me. I searched even the big hospitals in my country and they do not have facilities to run Antigen test. All what they say is, "We test for antibodies only". Doctors here say PCR test can not be used as stand alone solution to check HIV as they might give false-positive results.

I'm really confused :( What do you think about the antibody test I had after 33 days of the exposure ? If in case these were ARS symptoms, I have taken the antibody test 8 DAYS AFTER the onset of symptoms. Is this sufficient time period to detect antibodies ?

Again, my thanks for your time.

Many thanks for your reply.

Really I'm aware the fact that oral sex carries the lowest if not No risk. That's why I even did not think about this incident later. But when I developed some symptoms "without any known base", I got scared. You know mind is crazy which even thinks that I might become the first to get infected with HIV by oral sex :'( Can't stop these weird thoughts unfortunately.

Trust me, you had no risk from the encounter you mention. To get peace of your mind, do a hiv duo test now. It will give u good peace of mind. Also a pcr rna can help reassure you. But it is expensive.

The doctor told you the same answer that you'll find here:

and he told me "If you only engaged in oral sex, you have no risk, even if she was HIV Positive".

There has not been a case of a woman infecting a man during oral sex. Your symptoms are not related to HIV.

Btw, to add as a personal experience, I suffer from constant sore throat, like a month, if my GERD is in worst conditions than normally.

Stay well.
