Protected sex with African (Ghana) street hooker

Hi there docs.

I recently (17 days ago and counting) got drunk and went to a bar where I live in Oman. There were some hookers at the bar. I took one home with me. We had protected sex for about ten seconds. I was semi hard so I gave up having sex with her. The condom remained on an intact throughout. However, she then proceeded to give me a rimjob. I also fingered her and then masturbated.

I can only assume she is HIV positive. She told me after that her boss beats her and she had a burn scar on her face which was from her boss. I am new to this country and it’s awful that this happens. She seemed so happy at the bar.

My main concerns are should I be worried about:

1. Protected vaginal sex with HIV + person
2. Unprotected rimjob. She spat in my anus a few times as well as licking it for a few minutes.
3. My masturbation of myself with fingers that may have had vaginal fluid on them.
4. What happens if I rubbed my eye by mistake with her vaginal fluid?

I have booked into a private clinic in London on day 21 post exposure (this Friday). They run the duo test.  I will also test for syphilis, gonorrhoea, clamidia, and hepatitis A. Will that be conclusive?

This answers all of your HIV questions, and if you can think of any more just reread about the 3. You had zero risk therefore  testing is irrelevant to your situation because you had zero risk. HIV is a fragile virus, which is instantly inactivated in air and also in saliva which means it is effectively dead so it can't infect from touching, external rubbing or oral activities. It doesn't matter if you and they were actively bleeding or had cuts at the time either because the HIV is effectively dead.
Only 3 adult risks are the following:
1. unprotected penetrating vaginal with a penis
2. unprotected penetrating anal sex with a penis
3. sharing needles that you inject with.
The only way to get HIV is if you did one of the 3. The situation you describe is a long way from any of these 3.
Even with blood, lactation, cuts, rashes, burns, etc the air or the saliva does not allow inactivated virus to infect from touching, external rubbing or oral activities. Doctors have calculated the risk from what you describe to be less than that of being hit by a meteor, therefore no one will get HIV from what you did in the next 40 years of your life either. The above HIV science is 40 years old and very well established, so no detail that you can add to your encounter will change it from zero risk.
If you didn't have one of the 3 then you are just worrying about your own hiv theory - which is unrealistic for you to think that can become reality - so you should move on back to your happy life instead.
You can't get hiv when you used a condom. Cancel the hiv appointment and have fun instead. There is an std forum for that, however i doubt you have any std risk either.. Likely she is negative for hiv anyway, otherwise she wouldn't waste money on condoms because she uses them to protect herself from clients, not the other way around.

Also, when you say the saliva does not allow “inactivated virus to infect...” how do you know it is inactivated? Or do you mean that it is inactivated by the saliva itself and so in my case when I got a rim job even if the spit came from the back of her throat there is no way I could have been infected as the virus by that point was inactivated?

Many thanks for allaying my fears my friend. I am still going to spend 300 pounds sterling on my STI test for price of mind though, just to be sure, and in any event I should have one very so often even if I only do protected sex.  I also have a girlfriend and would never ever wish to hurt her.

I neglected to say she didn’t provide the condom, I did. She didn’t have any condoms with her. I think she is forced into her profession by a pimp. She told me she gets beaten and I saw a burn mark on her from her boss. It is sad. But it makes it more likely she has unprotected sex which entails more risk.

Thanks again. You are very kind to spend your time thinking and writing to me. Much appreciated.

A condom protects you from HIV transmission as long as the head is covered.  The only way adults get HIV is from unprotected vaginal or anal intercourse or sharing of IV drug needles.  Her spit would not spread the virus as enzymes in saliva render the virus inactive and air does the same.  There are no reported, documented proven cases of someone getting HIV from oral contact.  The same goes for her vaginal secretions on her fingers. Air will inactivate it.  The HIV virus is fragile and easily broken down.

So, you had zero risk.

Thanks Guitar Rocks. I’m getting a test anyway for good measure from a clinic in London, England. They do the duo test. I know I don’t need a test but for piece of mind I do. Will the duo be accurate after 21 days? Many thanks once again.
Presumably the spit and saliva doesn’t penetrate into the anus in any event?

Well, I'm not sure why you are asking if a test is accurate that you acknowledge you don't need because you didn't have a risk.  If you didn't have a risk, all tests will be negative whenever you take them.

But a duo test is accurate at 28 days in general.

Slightly concerned as I have a cold - maybe my immune system is down because of my anxiety

Last time. You didn't have a risk.  The ONLY risks are having UNprotected vaginal or anal intercourse or sharing IV drug needles.  Any symptoms or this or that you have is NOT hiv related as your activity including the saliva you are very worried about (and has been explained to you it has enzymes that inactivate hiv) --  you did not have a risk.  Colds happen to everyone.

Move on, you don't need to worry about HIV.

Thanks Guitar Rocks. I know it’s my paranoia. Much appreciate the advice and to good work you do. Enjoy the evening or day whatever time it is where you are
If you know it's paranoia, you must see a therapist to take care of it. As suggested to you by other members, you didn't posses risk from your incident and do not need to test. It's time to move on for you.
I took the Duo test and I was negative. Time to move on.
