Sex - Appropriate Condom Use

Hello everyone,

I had protected sex with this women around 3 months ago. I know protected sex is safe sex, if the condom was used correctly.

Now what is making me nervous is that I removed the condom to ejaculate on her breasts. Now does that put me at risk of HIV or any other STD's? Is this deemed incorrect use of a condom?
