Vaginal sex, condom broke and started PEP after 24 Hours. Am I high risk for HIV?

Very sad story. Met a girl in strip club, after few days I (44 years old) had a protected sex with that girl. But my condom was broke in middle and my top exposed for 20 seconds - 1 min max. I stop the sex and clean my penis with soap water. Penis is dry and no fluids have been noticed. I asked girl about her sexual health. She told me that she has 3 kids, she delivered her last kid 6-7 months back. She also mention that she had complete tests while she had a pregnancy and all tests are OK. She said, she is a family person and she is doing stripping because easy job (However, I have no evidence to prove that).

Next day Call the city health, explained exposure and requested PEP and they said PEP is not advised. However, I went to local clinic and requested PEP and started Truvada my 1st day (incident happened 24 hours back).

Local Doctor gave Truvada (PEP) and other antibacterials drugs as well.

I feel so guilty and I have no sex with any other girl other than my wife. I am not sexually active and this is so unfortunate event for me.

Am I pose risk here? what are my next steps? Please advise. I am so panic and not getting enough sleep by thinking the same exposure.

You can't test while taking PEP, if that is what you were asking. Well, I mean, you can, but your test result will come back undetectable (negative) but that is not going to tell you whether or not you've been infected. If you were asking if you could test 10 days after you've completed PEP, I've addressed the post PEP testing protocol below.  

Successfully completing PEP is a very good predictor of being HIV negative, but you MUST take it for the full 28 days. If you are having side effects, your doctor can give you medications to help.

The recommendations for post-PEP HIV testing are six weeks, three months and six months from the date of exposure. If you Google post-PEP testing schedules, you will find many different opinions. I know you are anxious to have an answer, but if you want the true answer, you wait and you use condoms until the final result.

Thank you again for helping me in this panic situation. I did talk to the girl and explained my panic situation and she was laughing at me as soon as I said HIV. She is well aware of HIV and She is kindly accepted me to go through ‘Ora-Quick test’. We purchased Ora Quick from Walmart and she came out negative. One thing I learn that it is not fair to accuse someone with HIV without knowing facts (I have to apologize big time to her and she kindly accepted my apology).  
I am so paranoid and started PEP and this is the 4th day. However, I will continue PEP without panic.
I learn my lesson!!!
@ RubyWitch I am almost last 7 days of my PEP and no major side effects. I will continue.

I forget to ask you that...Is  it Truvada alone is effective? I am taking only Turvada (Tenofovir 300mg / Emtricitabine 200mg)   tabs for 28 days. Please advise.

The condom broke and you continued sex for up to a minute. This puts you at enough of a risk you must either test or begin PEP.......IF you are within the 72 hour window period.

Those are your choices.

Thank you Doc. I start taking PEP after 24 hours of exposure. I feel little relax after reading Truvada confirmations. I have one last question...Do you recommend HIV-1 RNA test after 10 days? I would like to clear from this tension soon. Please help.

First and most importantly, you MUST complete the full 28 day treatment plan for PEP to hopefully prevent infection.

PEP drugs are very powerful and can make you feel very sick, which is why so many people stop taking it mid way through treatment. Your doctor can prescribe medications to help with the side effects.

After you have completed PEP, you will need to test for HIV at 4-6 weeks, 3 months and 6 months for a 100% conclusive result.

Because PEP is not 100% effective, you should continue to use condoms with sex partners while taking PEP.
