First of all, I'd like to say that a CSW is not necessarily at a higher risk for HIV than anyone else. ANYONE can be HIV+, no matter what occupation, ethnicity, age, gender or sexual orientation. It is the ACTIVITY that poses an HIV risk.
Regarding your question, a broken condom carries an HIV transmission/acquisition risk that is the same as that of unprotected sex. Your exposure WAS very brief, which would decrease, but not completely eliminate, the transmission risk.
I have no doubt whatsoever that you will find doctors out there that will tell you your "risk" was virtually non-existent and you have no need to test.
They may even have a handy chart of algorithms that will "prove" why you don't need to test.
Perhaps they are absolutely correct.
I believe that a risk is a risk no matter how quick it was. And we all know that unprotected anal sex is THE riskiest sex out there.
I would recommend that if this exposure happened LESS than 72 hours ago, go to the ER and begin PEP treatment.
If it has been longer than 72 hours, that window is no longer open. I then recommend you test at 28 days post this exposure with a 4th generation DUO test. You can test again at 8 weeks if you want and this test will give you a fairly good indication of your status, but you will still need to have a 12 week confirmatory test for 100% conclusive results.
