A nuclear stress test can miss blockages in any artery, large or small. However, Nuclear tests are normally performed after an Angiogram which shows no blockages, to see how well the cardiac muscle is Oxygenated.
The problem is, if your body opens collateral vessels on the Heart, it will increase blood flow to the other side of blockages, giving the impression on a nuclear scan that everything is fine.
