Normal Pulse and BP at the time of MI?

Is it possible that the pulse and blood pressure are normal when someone is suffering MI? If yes, What could be MI symptoms in such a situation?

You get called for a patient that is 50 years old and complaining of trouble breathing you arrive to their home and you obtain full set of vitals you find their pulse to be 115 blood pressure is a 114/80 respiratory rate is 28 and their skin is a little sweaty. Which of the following vital signs are abnormal ?

I have good EF 50% - 59%.  I have massive sweat.  sweat all year long (4 seasons).  The sweat 4-6 times a night can wake me up during the sleep.  No doctor found anything wrong.  Just let me sweat, it's not life threatening so no one care.

Now my LAD has been fully opened, things have changed again. After eating any type of food, I get hot, usually to the point of sweating. This can even be after eating half a slice of toast, but never from drinking, even hot beverages. My HR jumps to between 100-110 and I feel exhausted. I don't get angina, but it feels just like I've been drugged and I really can't keep my eyes open, no matter how hard I try. I fall aspeep for anywhere between one and three hours.

The oxygen demand by digestive system keeps varying based on some factors. For same type of food the demand can vary in a wide range. This is reflected in the pulse rate after I take the meals. Sometimes the pulse is calm after the meals and throughout the digestion process but sometimes, for the same type of food and same routine, the pulse rate is markedly higher after the meals. The pulse prior to meals is generally same.

I have no idea what factors influence the digestive system to demand very high oxygen at some time and relatively lower demand at some other. I do sweat after the meals sometimes, but not always.

In my case the EF is 30% so sweating is understandable, as heart may have to work harder. But in a high EF person, can heart related sweating be tiggered by meals?

Ed's digestive system created a greater need for blood/oxygen and the greater demand required less blood to the peripherals and still provide blood to the vital organs.  Less blood to the peripherals and more to the vital organs constricts vessels and higher blood pressure.  Apparently, that compensation was not sufficient and took away good blood flow to the heart not withstanding an increase in the heart rate.

my pulse was around 120-140 and bp was virtually normal, but I did sweat a lot. Each episode lasted around 20-30 minutes. The day before my first MI I was digging a 2 metre deep trench in clay using a spade. No problems. Also, after the MI stopped (20-30mins), I was able to carry on doing anything physical. All I can say is, the digestive system must require lots of oxygen.

My pulse was 115 bpm (2-3 days) sustained while I was having a heart attack (stunned heart cells) that led to congested heart failure. There may be a misunderstanding regarding heart attack and myocardial infraction.  An MI damages (necrotic or stunned) heart cells and CO is reduced..  Some people classify a heart attack to be acute chest pains from ischemia.

Did you measure pulse rate or blood pressure? During my MI in 2007 I had high BP and high pulse.

In my case stated above, I had no problems during walk prior day.
If not MI, could this heart heaviness post meal, be a sign of angina?

Same as my situation, it was food digestion which overloaded the heart causing MI. My ecg also looked normal. However, my troponin was high, so fingers crossed."

"Having had" an MI may not cause any noticable changes in heart rate and blood pressure when the system has time to compensate and with medication, or heart cells were stunned, located in area that is not critical (ie septum or distal part of the heart), etc.

How can it not effect vital signs? There has to be a misunderstanding!

The system compensates when there is low cardiac output.  MI damages heart cells and impedes left ventricle contractions.  Impeded left ventricle reduces cardiac output.  ....A Source: "Low cardiac output increases heart rate and blood pressure. The pulse rate may increase to compensate for a decreased cardiac output, producing a tachycardia. If the low cardiac output causes ischemia to the cardiac tissue, though, a bradycardia can occur" (with cardiac arrest the blood pressure willl be low).  

Source: "The blood pressure is determined by the rate of blood flow produced by the heart (cardiac output), and the resistance of the blood vessels to blood flow".

A doctor works at the country side told me..... MI won't affect the blood pressure.  Many MI patient remain in normal range of blood pressure.  She didn't mention about the heart rate (pulse).

What a relief reading your reply. I was not told all this by my Doctor who asked me to get an ECG and blood tests.

I had some heavyness on left side of chest and some stiffness in left neck, which lasted for about three hours. I also had to take long breaths. This started two hrs after my breakfast and morning dose of betablocker (carvidilol). But my pulse and BP were normal all through. I had no symptoms previous day when I went for my regular walk.

My ECG was normal two hrs after this incident. Awaiting results of my blood test. I was really worried.

Now I am sure it was not cardiac. Thanks.

No, it is not possible to have no change HR or BP.

An MI well decrease the cardiac output (amount of blood pumped for each minute), and the heart rate will increase to compensate.  A decrease of CO will cause the system to react the same as if there is blood loss (bleeding). example, when a person is bleeding, blood volume and thus blood pressure decrease. There are compensatory mechanisms to prevent blood pressure from decreasing too much: The heart rate increases, increasing the amount of blood pumped; the veins constrict, reducing their capacity to hold blood; and the arterioles constrict, increasing their resistance to blood flow (higher blood pressure).
